notice_summary_english: Northern Queensland Primary Health Network (NQPHN) invites applications from service providers interested in supplying a multidisciplinary allied health team using a ’work-in, work-out' model of care for underserviced communities within the NQPHN western corridor. This tender covers rural and remote areas of the Townsville Hospital and Health Service (HHS) region. Funded services will be delivered through block-funding and focus on the care of chronic conditions in alignment with the Chronic Conditions Management Plans developed by local general practitioners. This approach will also create opportunities for applicants to combine block-funded appointments with privately billed sessions.
