INVITATION FOR BIDS

CROATIA HEALTH SYSTEM PROJECT

Loan No. IBRD-4513-0 HR

Supply of Medical Equipment for Koprivnica Pilot Hospital
and Primary Health Care Centers

Contract Identification No: IFB No. II-1-2-ICB

1. This invitation for bids follows the general procurement notice for this project that appeared in Development Business, of August 31, 1999, update of September 30, 2002, and September 30, 2003.

2. The Republic of Croatia has received a loan from the International Bank for Reconstruction and Development toward the cost of Health System Project, and it intends to apply part of the proceeds of this loan to payments under the contract no. II-1-2-ICB for Supply of Medical Equipment for Koprivnica Pilot Hospital and Primary Health Care Centers. The current closing date for the loan is June 30, 2005.

3. The Ministry of Health of the Republic of Croatia now invites sealed bids from eligible bidders for supply of Medical Equipment for Koprivnica Pilot Hospital and Primary Care Health Centers consisting of the following Lots:
Lot 1. Operating Theatre Equipment
Lot 2. Surgical, Intensive Care and Pathology Related Equipment
Lot 3. Laboratory Equipment – A (Centrifuges, Sterlization Equipment and Similar)
Lot 4. Laboratory Equipment – B (Anticillary Lab Equipment)
Lot 5. Laboratory Equipment – C (Analyzers)
Lot 6. Medical Endoscopy Equipment
Lot 7. Ventilators, Medical Investigation and Therapeutic Equipment
Lot 8. Radiology Imaging Equipment
Lot 9. General Practice Medical Equipment
Bidders are invited to bid for one lot, a combination of lots, or all lots. Bid evaluation will be carried out on a per lot basis. Bids for incomplete lots will be rejected.

4. Bidding will be conducted through the international competitive bidding procedures specified in the World Bank’s Guidelines: Procurement under IBRD Loans and IDA Credits, and is open to all bidders from eligible source countries as defined in the Guidelines (January 1995; Revised January and August 1996, September 1997, January 1999).

5. Interested eligible bidders may obtain further information from the Project Management Unit and inspect the bidding documents at the address given below from 9:00 a.m. to 12:00 p.m. Zagreb time, on working days.

6. A complete set of bidding documents in English language may be purchased by interested bidders on the submission of a written application to the address below and upon payment of a nonrefundable fee of HRK 610, or USD 100. The method of payment will be direct payment to specified account no. 2390001-1500001797 for payments in HRK or account no. 7020-840-4489 in the Raiffeisen Bank Austria d.d. Zagreb (Swift code: RZBHHR2X) for payments in USD. The document will be sent by airmail for foreign delivery and surface mail for local delivery.

7. Bids must be delivered to the address below at, or before 14:00 p.m. Zagreb time, December 3, 2004. All bids must be accompanied by a bid security of HRK 60.000 (sixty thousand) for Lot 1; HRK 35.000 (thirty five thousand) for Lot 2, HRK 25.000 (twenty five thousand) for Lot 3; 25.000 (twenty five thousand) for Lot 4; 28.000 (twenty eight thousand) for Lot 5; HRK 22.000 (twenty two thousand) for Lot 6; HRK 62.000 (sixty two thousand) for Lot 7; HRK 30.000 (thirty thousand) for Lot 8; and HRK 15.000 (fifteen thousand) for Lot 9, or an equivalent amounts in a freely convertible currency. For a bid consisting of more than one lot, the bid security can be submitted either separately for each lot, or one bid security covering the total amount for all submitted lots. Late bids will be rejected.

8. Bids will be opened in the presence of the bidders’ representatives who choose to attend at the address below at 14:00 p.m. Zagreb time, December 3, 2004.
