INVITATION FOR BIDS

CROATIA HEALTH SYSTEM PROJECT

LOAN NO. 4513-0-HR

Supply of Telecommunication/Telemedicine System for Support of Emergency Medical Service on Selected Croatian Islands

Contract Identification No: IFB No. II-13-3-ICB

1. This invitation for bids follows the general procurement notice for this project that appeared in Development Business, of August 31, 1999, update of September 30, 2002, and September 30, 2003.

2. The Republic of Croatia has received a loan from the International Bank for Reconstruction and Development toward the cost of Health System Project, and it intends to apply part of the proceeds of this loan to payments under the contract no. II-13-3-ICB for Supply of Telecommunication/telemedicine System for Support of Emergency Medical Service on Selected Croatian Islands. The current closing date for the loan is June 30, 2005.

3. The Ministry of Health of the Republic of Croatia now invites sealed bids from eligible bidders for supply of Medical Equipment for Koprivnica Pilot Hospital and Primary Health Centers consisting of the following items in one Lot:

LOT 1:

(i) Videoconference equipment
(ii) 12-channel tele-ECG device
(iii) Digital camera (6 Megapix)
(iv) Computer with peripheral instruments
(v) Laser printer
(vi) Television set

The Lot with all items listed above comprise a complete system. Bidders are invited to submit bids for the whole Lot (i.e. complete system). Incomplete bids (i.e. only for some of the items) will be rejected as non responsive.

4. Bidding will be conducted through the international competitive bidding procedures specified in the World Bank’s Guidelines: Procurement under IBRD Loans and IDA Credits, and is open to all bidders from eligible source countries as defined in the Guidelines (January 1995; Revised January and August 1996, September 1997, January 1999).

5. Interested eligible bidders may obtain further information from the Project Management Unit and inspect the bidding documents at the address given below from 9:00 a.m. to 12:00 p.m. Zagreb time, on working days.

6. A complete set of bidding documents in English language may be purchased by interested bidders on the submission of a written application to the address below and upon payment of a nonrefundable fee of HRK 610, or USD 100. The method of payment will be direct payment to specified account no. 2390001-1500001797 for payments in HRK or account no. 7020-840-4489 in the Raiffeisen Bank Austria d.d. Zagreb (Swift code: RZBHHR2X) for payments in USD. The document will be sent by airmail for foreign delivery and surface mail for local delivery.

7. Bids must be delivered to the address below at, or before 14:00 p.m. Zagreb time, December 6, 2004. All bids must be accompanied by a bid security of HRK 90.000 (ninety thousand), or an equivalent amounts in a freely convertible currency. Late bids will be rejected.

8. Bids will be opened in the presence of the bidders’ representatives who choose to attend at the address below at 14:00 p.m. Zagreb time, December 6, 2004.
