1.Declaratie pe propria raspundere-Formularul 12 A, întocmit în baza art.180 din OUG 34/2006.
2.Declaratie pe proprie raspundere-Formularul 12 B, întocmit în baza art.181 din OUG 34/2006;
3.Prezentare de certificate de atestare fiscala privind plata taxelor si impozitelor la bugetul general consolidat (DGFPS) si cel local, în original sau copie legalizata , valabil la data depunerii ofertei

În cazul în care în tara de origine sau în tara în care este stabilit candidatul nu se emit documente de natura celor prevazute mai sus, sau respectivele documente nu vizeaza toate situatiile prevazute de art.180 si 181 din O.U.G. nr. 34/2006, candidatul va prezenta o Declaratia pe propria raspundere sau o declaratie autentificata în fata unui notar, a unei autoritati administrative sau judiciare sau a unei asociatii profesionale care are competente în acest sens

1) Certificate constatatoare emise de Oficiul Registrului Comertului, conform Fisei de date
2) Autorizatie de functionare de la BNR, conform Fisei de date

III.2.2) Economic and financial capacity
Information and formalities for evaluating if requirements are met:
1.Cifra de afaceri medie anuala calculata pentru anii financiari 2007, 2008 si 2009 de 10.000.000 lei;
Cursul ce va fi utilizat pentru conversia leu-euro este cursul valutar comunicat de BNR în ultima zi bancara a fiecarui an.
Cifra de afaceri medie anuala se va determina ca medie aritmetica a cifrelor de afaceri obtinute de candidat în anii financiari precizati.
Persoanele juridice înfiintate de mai putin de trei ani, vor prezenta cifra de afaceri doar pentru anii scursi de la data înfiintarii, însa vor lua în considerare toate celelalte cerinte privitoare la acesti indicatori, respectiv cifra de afaceri medie anuala sa fie de 10.000.000 lei.

Cerinte obligatorii:
1). Experienta similara :
a.Candidatul va prezenta declaratia si o lista a principalelor servicii de acordare credite în conditii similare celor solicitate prin caietul de sarcini, prestate în ultimii 3 ani, continând valori, perioade de prestare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati - Formular 12 E
Candidatul înfiintat de mai putin de 3 ani, va prezenta lista principalelor servicii de acordare de credite prestate de la infiintare.
Nota:Toate documentele emise în alta limba vor fi însotite de traducere autorizata si legalizata în limba româna.Cerinte obligatorii:
Candidatul va prezenta:
1). Declaratia si lista cuprinzând subcontractantii (daca este cazul), conform Formular 12G precum si acordurile de subcontractare;
Ofertantii sunt obligati sa precizeze:
- partea/partile din contract pe care urmeaza sa o/le subcontracteze;
si
- datele de recunoastere a subcontractantilor propusi;
Toti subcontractatii trebuie sa completeze cu propriile date formularul cu informatii generale privind societatea - Formular B1.
2). Acordul de asociere (în cazul ofertei depuse în comun), încheiat în vederea participarii la procedura de achizitie publica.
În cazul în care oferta comuna este declarata câstigatoare se solicita legalizarea asocierii si a drepturilor si obligatiilor partilor.
