Certificat fiscal privind plata taxelor si impozitelor la bugetul general consolidat - ANAF valabil la data depunerii ofertelor.
Certificat fiscal privind plata taxelor si impozitelor la bugetul local valabil la data depunerii ofertelor.
Cerinta obligatorie: completarea unei Declaratii pe proprie raspundere privind neîncadrarea în prevederile art. 691 din OUG nr.34/2006 cu modificarile si completarile ulterioare.
Pentru persoane juridice/fizice romane:
- Certificat constatator emis de Oficiul Registrului Comertului
- Certificat unic de înregistrare emis de Oficiul National al Registrului Comertului
- Cerinta obligatorie pentru persoane juridice/fizice straine - documente edificatoare similare cu cele ale Persoanei juridice/fizice române care dovedesc o forma de înregistrare/atestare ori apartenenta din punct de vedere profesional, în conformitate cu prevederile legale din tara în care ofertantul este rezident. Documentele vor fi prezentate cu traducerea legalizata.

Information and formalities for evaluating if requirements are met:
Pentru persoanele juridice:
- Bilantul contabil la data de 31.12.2009 vizat si înregistrat de organele competente în conditiile art. 185 din OUG nr. 34/2006 cu modificarile si completarile ulterioare.
- declararea cifrei de afaceri pe ultimii 3 ani - Fisa de Informatii generale, Formular 4 - se completeaza; (cifra de afaceri medie anuala, pe ultimii 3 ani, sa fie de minim 250.000 lei).
III.2.3) Technical capacity
Information and formalities for evaluating if requirements are met:
a) experienta similara si expertiza în domeniul formarii profesionale
Lista principalelor prestari de servicii în ultimii 3 ani.
- Se completeaza Formular 12 E cu prestari de servicii de formare profesionala a caror valoare depaseste 100.000 lei;
- Se vor prezenta documente relevante, dupa caz, conform cerintelor specificate în Caietului de sarcini;
Cerinta minima obligatorie - cel putin 1 contract de servicii de formare profesionala/documente fiscale justificative cu valori mai mari de 200.000 lei.
- Recomandari din partea altor beneficiari/clienti - cel putin 1 recomandare (copii conforme cu originalul);
Cerinta minima obligatorie - recunoasterea europeana a certificarii acordate programului - copie conform cu originalul dupa documente relevante ( ex. : acord de parteneriat / contract cu institutia care acorda certificare europeana, acord de afiliere din organizatii europene de formare, copie dupa diploma/certificatul cu recunoastere europeana acordat)

- un manager de training
- un responsabil de formare

- calitatea de participant la procedura - Formular 12C persoana desemnata de operatorul economic sa îi reprezinte interesele în cadrul sedintei de deschidere a ofertelor, va cunoaste continutul documentatiei si va putea lua decizii în numele acestuia. De asemenea, va prezenta un act de identitate.
