Autorizatia in vigoare eliberata de Comisia de Supraveghere a Asigurarilor din Romania, in copie; declaratie si lista principalelor prestari de servicii in ultimii 3 ani. ofertantul trebuie sa faca dovada indeplinirii in ultimii 3 ani (2007-finalizat mai 2010)a unui contract cu obiect asemanator celui pentru care se oferteaza, a carui valoare initiala a fost de minim 126.000 lei.ofertantul va anexa copia contractului prezentat ca experienta similara si va prezenta certificare de buna executie; recomandare din partea beneficiarului contractului
