2. Declaratie privind neincadrarea in situatiile prevazute la art. 181 din OG 34/2006.
3. Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat (sanatate, pensii, somaj), eliberat de Directia Generala a Finantelor Publice si Controlul Finantelor de Stat.
4. Certificat privind plata taxelor si impozitelor locale - eliberat de Primaria locala.
5. Certificat constatator, emis de Oficiul Registrul Comertului de pe langa Tribunalul teritorial.

a) lista a principalelor livrari de produse similare efectuate in ultimii 3 ani, continand valori, perioade de livrare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati. Livrarile de produse efectuate se confirma prin prezentarea de recomandari din partea autoritatii ori a clientului privat beneficiar. Se va completa si depune in original, Formularul C1 din Sectiunea Formulare.Prin informatiile furnizate in Formularul C1, firma ofertanta trebuie sa prezinte unul/mai multe contracte similare (incheiate si finalizate in ultimii 3 ani). Suma valorilor incluse in contractul/contractele prezentat/prezentate de catre operatorul economic ca dovada a experientei lui similare, sa fie mai mare de 320.000 lei, echivalentul a 80.000 euro. (Se vor anexa in copie contractul/contractele la care se face referire ). Se vor anexa recomandarile emise de beneficiarii la care se face referire in Formularul C1, ele fiind pentru contractele similare prezentate in copie
b) declaratie pe proprie raspundere referitoare la echipamentele tehnice si la masurile aplicate in vederea asigurarii calitatii, precum si, daca este cazul, la resursele de studiu si cercetare.
c) declaratie pe proprie raspundere referitoare la personalul/organismul tehnic de specialitate de care dispune sau al carui angajament de participare a fost obtinut de catre ofertant, in special pentru asigurarea controlului calitatii.
d) certificate sau alte documente emise de organisme abilitate in acest sens, care sa ateste conformitatea produselor.
e) mostre, descrieri si/sau fotografii.
f) autorizatie de livrare, in original sau copie legalizata (numai pentru comercianti).
