1.Declaratie privind situatia personala completata în conformitate cu Anexa 10 din Sectiunea Formulare. - în original;
2.Declaratie pe propria raspundere ca nu se afla în nici una din situatiile prevazute la Art.5 alin.1 din Legea 21/1996 privind concurenta completata în conformitate cu Formularul N din Sectiunea Formulare - în original;
3.Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat (sanatate, pensii, somaj), eliberat de Directia Generala a Finantelor Publice si Controlul Finantelor de Stat -valabil la data licitatiei, în original sau copie legalizata.
4.Certificat privind plata taxelor si impozitelor locale - eliberat de Primaria locala- valabil la data licitatiei, în original sau copie legalizata.
5.Pentru persoane juridice române: - Certificat constatator in original sau copie legalizata (valabil la data licitatiei) emis de Oficiul Registrului Comertului de pe lânga Tribunalul teritorial;
6.Pentru persoane juridice straine: - documente edificatoare care sa dovedeasca o forma de înregistrare ca persoana juridica atestare ori apartenenta profesionala, în conformitate cu prevederile legale din tara în care ofertantul este rezident - copie legalizata + traducere autorizata.
