Enclosed is the Request for Quotation (RFQ) for the Department of Health and Human Services, Office of Public Health and Science, Office of Regional Health Administrator to provide research on healthcare-associated infections. This is to include the planning of focus groups, developing questions for consumers to asses their ability to make informed health care decisions, conduct preliminary research of existing healthcare-associated infections websites, and a final report. Please submit your proposal in accordance with Attachment A � Statement of Work and Attachment C � Instructions to Quoters and Evaluation Criteria. Awards shall be based on the Quoter�s technical understanding of the requirement and the reasonableness of the proposed price. FAR Subpart 13 Simplified Acquisition Procedures shall be utilized. The quote is due on or before September 15, 2010 at 1:00PM Eastern Standard Time. All quotes are to be submitted electronically to Isaac Bellamy at Isaac.bellamy@psc.hhs.gov. Proposals shall be written in English and separated into a Technical Volume and a Business Volume. Additionally, one complete copy of Attachment B, �Representations, Certifications, and Other Statements of Quoters� and the cover page of the Request for Quotation, Standard Form 18, with Block 13 signed and Blocks 14, 15, and 16 completed. In order to receive an award, a contractor must be registered in the Central Contractor Registration database. Please read the RFQ in its entirety. Questions must be submitted in writing to the attention of Isaac Bellamy. No telephone calls concerning questions regarding the RFQ will be accepted. All questions must be sent via email to Isaac.bellamy@psc.hhs.gov and received by 1:00PM EST on September 8, 2010.
