This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The primary purpose of this contract is to provide a network of EFMP respite care providers who will provide temporary rest periods for Family members responsible for regular care of persons with disabilities. Care may be provided in the EFMP respite care users home, other facility or setting. Eligibility is determined based on enrollment in EFMP by the Government Representative. The RFQ is attached to this notice.
