Information and formalities for evaluating if requirements are met:
1) Declaratie de eligibilitate (Formular F3) si declaratie privind neincadrarea in situatiile prevazute la art. 181 din OUG 34/2006 (Formular F4).
2) Certificate constatatoare privind indeplinirea obligatiilor exigibile de plata a impozitelor, taxelor si contributiilor catre bugetele componente ale bugetului general consolidat, valabile la data deschiderii ofertelor.
III.2.2) Economic and financial capacity
Information and formalities for evaluating if requirements are met:
Ofertantii trebuie sa prezinte ultimul bilant contabil, vizat si inregistrat de organele competente.
In cazul in care ofertantul isi demonstreaza situatia economica si financiara invocand si sustinerea acordata, de catre o alta persoana, atunci acesta are obligatia de a dovedi sustinerea de care beneficiaza, de regula prin prezentarea unui angajament ferm al persoanei respective, prin care aceasta confirma faptul ca va pune la dispozitie ofertantului resursele financiare invocate. Persoana ce asigura sustinerea financiara nu trebuie sa se afle in situatia care determina excluderea din procedura de atribuire conform prevederilor art. 180 si 181 din OUG 34/2006.
Atunci cand un grup de operatori economici depune oferta comuna, situatia economica si financiara se demonstreaza prin luarea in considerare a resurselor tuturor membrilor grupului. In cazul in care grupul beneficiaza de sustinerea financiara a unei/unor terte persoane, situatia economica si financiara se demonstreaza in conditiile prevazute mai sus.
III.2.3) Technical capacity
Information and formalities for evaluating if requirements are met:
1). Ofertantii trebuie sa prezinte dovada ca in ultimii 3 ani au indeplinit si finalizat cel putin un contract care a avut ca obiect prestarea unor servicii similare cu acelea cuprinse in caietul de sarcini, a carui valoare este mai mare sau egala cu 6.900 lei. In acest sens, ofertantii vor completa lista principalelor lucrari similare executate in ultimii 3 ani (Formular F5). Acestea se vor certifica prin copii ale urmatoarelor documente: contractele cu actele aditionale respective, daca este cazul, procese verbale de receptie si recomandari din partea beneficiarilor.
2). Declaratie pe proprie raspundere privind asigurarea personalului de conducere in prestarea serviciilor, avand studii de specialitate, asigurarea unui responsabil tehnic privind indeplinirea obiectului contractului, care trebuie sa fie atestat in domeniul serviciilor care urmeaza a fi prestate si care trebuie sa aiba experienta in realizarea serviciilor care fac obiectul contractului (Formular F6).
Declaratia va fi insotita de CV-uri ale persoanelor de conducere responsabile direct de indeplinirea contractului, precum si ale oricaror specialisti cu sarcini "cheie" in indeplinirea contractului, copii ale autorizatiilor si atestatelor profesionale in cauza, valabile la data deschiderii ofertelor.
3). Declaratie referitoare la dotarea tehnica utilizata pentru indeplinirea contractului (Formular F7).
4). Autorizatie de mediu valabila la data deschiderii ofertelor.
