Declaratie privind eligibilitatea, neincadrarea in prevederile art.181, certificat privind achitarea obligatiilor la bugetul local si cel de stat, certificat constatator emis de ORC. Se califica ofertantii care prezinta certificatele care sa ateste lipsa datoriilor fiscale si sociale emise de DGFP si de primariile pe raza carora ofertantul isi are sediul social si punctele de lucru implicate in derularea executiei lucrarilor in original / copie legalizata, valabile la data deschiderii ofertelor

Se califica ofertantii care au profituri nete pozitive la 31.12.2009, 31.12.2008 si 31.12.2007
Se califica ofertantii cu cifra de afaceri medie anuala pe ultimii 3 ani fiscali de cel putin 17.500.000 lei
Se califica ofertantii care prezinta un cash-flow realist, corect, si corelat cu cu graficul de executie propus, cu echipamentele din dotare, cu personalul alocat, cu resursele financiare proprii si termenele de rambursare a cheltuielilor de catre APDRP (3 luni de la data depunerii cererii de plata), precum si o scrisoare bancara de confirmare a fondurilor, cu o valoare suficienta pentru a asigura cash-flow-ul propus (minim 20 % din valoarea estimata a contractului cu TVA).

Se califica ofertantii care in ultimii 5 ani au semnat si finalizat:
-un contract de lucrari de constructii privind infrastructura de apa si/sau apa uzata a localitatilor in valoare de minim 1.300.000 lei fara TVA
-un contract de lucrari de constructii de drumuri in valoare de minim 3.100.000 lei fara TVA
-un contract de lucrari de constructii de cladiri in valoare de minim 1.700.000 lei fara TVA.
-un contract de furnizare de autovehicule in valoare de minim 1.200.000 lei fara TVA.
Data de referinta pentru calcularea ultimilor 5 ani, este data sedintei de deschidere a ofertelor.
Nota: In cazul in care contractele prevad si servicii sau alte lucrari decat cele solicitate ca experienta similara, se va prezenta o detaliere a contractului, insusita de beneficiar, din care sa reiasa valoarea lucrarilor similare solicitate (centralizatoare F1, F2, procese verbale, oferte, etc)
Se califica ofertantii care prezinta contracte, procese verbale de receptie fara obiectii incheiate in termenele contractuale, si recomandari pozitive din partea beneficiarilor pentru toate lucrarile declarate in Formularul C3. In cazul contractelor de subcontractare sau in asociere, se prezinta atat documentele aferente contractului de subcontractare cu antreprenorul general cat si documentele aferente contractului dintre antreprenorul general si beneficiarul final. Toate documentele vor fi datate, semnate si stampilate
