Declaratie privind lista principalelor prestari de servicii in ultimii 3 ani, (formularul 12E). Declaratia va fi insotita de certificate/contracte/ documente emise sau contrasemnate de o autoritate ori de catre clientul privat beneficiar)
Declaratie care contine informatii privind spatiul unde se va desfasura activitatea de formare profesinala, dotarile specifice, echipament tehnic necesar desfasurarii contractului. Se va prezenta formularul 12H ( declaratie privind resursele tehnice) si de asemenea , copie de pe actul de proprietare sau de inchiriere a spatiului unde se va desfasura activitatea de formare profesionala.
Declaratie privind efectivul mediu al personalului angajat si al cadrelor de conducere ( formularul 12I) insotit de lista formatorilor care vor participa la desfasurarea programului de formare profesionala si care vor asigura pregatirea teoretica si instruirea practica, pe discipline, pentru meseria solicitata si de CV-ul fiecarui fpormator, intocmit in conformitate cu prevederile HG nr. 1021/25.06.2004, in care sa detalieze experienta in specialitatea corespunzatoare programului de formare si , eventual sa se precizeze daca are experienta in pedagogia adultilor; diplomele si certificatele din care sa reiasa ca detin specializarea ceruta de programul de formare ( copii).
