- se solicita constituirea garantiei de participare: 8.820,00 lei si a garantiei de buna executie: 5% din pretul total al contractului de funizare în Lei, fara TVA.

-Declaratie privind eligibilitatea (Anexa nr.1);
-Declaratie privind neîncadrarea în prevederile art. 181 din O.U.G. nr. 34/2006 (Anexa nr.2).

-pt.persoane juridice/fizice române: copie legalizata sau original, certificat constatator (extras de registru) eliberat de Oficiul Registrului Comertului de pe lânga Tribunalul competent teritorial ulterior datei de 08.06.2010, sau,
-pt.persoane juridice/fizice straine: documente edificatoare, traduse în limba româna si legalizate, valabile la data depunerii ofertei, care sa dovedeasca forma de înregistrare ca persoana fizica/juridica sau de înregistrare/atestare ori apartenenta din punct de vedere profesional si sa includa persoanele care reprezinta entitatea în relatiile cu tertii, în conformitate cu prevederile legale din tara în care ofertantul este rezident si obiectul de activitate al ofertantului.

Conform pct.IV.3.de la Sectiunea 1 din Documentatia de atribuire ofertantul va prezenta:
- Declaratie/lista principalelor livrari de produse efectuate în ultimii 3(trei)ani (Anexa nr.3) în care enumera unul/mai multe contracte încheiate si finalizate în ultimii 3(trei) ani, care au avut ca obiect furnizarea unor produse similare celor care fac obiectul prezentei proceduri si a carui/caror valoare cumulata este de cel putin 880.000,00 lei, fara TVA, însotita obligatoriu de certificate/documente constatatoare emise si contrasemnate de beneficiarii contractelor mentionate, care sa contina informatii referitoare la îndeplinirea obligatiilor contractuale;
- Documente edificatoare, în copie sau original, însotite de traducere autorizata în limba rom?na (daca este cazul), care sa-i ateste dreptul de a comercializa produse de tipul celor care fac obiectul procedurii de atribuire si pentru care a depus oferta.
