Note: Supply of 1) Tricycles : Local / imported, 2) Crutches Elbow crutches : Aluminium, 3) Walking Aids : Large / Small, 4) Artificial Limbs : Artificial hands (plastic), Artificial legs (plastic / socket/ rubber legs with aluminum body), 5) Calipus Shoes.
Refundable Tender deposit : Rs. 20,000/=.
Payment of a Non-refundable fee of Rs. 500/=.

The Chairman of the Procurement Board will receive bids from well reputed institutions and individuals for the supply of equipment mentioned in the following schedule.
Bidding forms could be collected upon Payment of a Non-refundable fee of Rs. 500/= and a refundable deposit amount as described above to Uva Provincial Social Services Department at R.H. Gunawardana Mawatha, Badulla.
Sealed bidding documents inserted in two separate envelopes as 'Originals' and 'Duplicate' should be sent by registered post to the following address to receive before 2.00 p.m. on 31,03.2010 stating "Quotations for Supply of equipment required for differently able persons" at the top left hand corner of the envelopes or be deposited in the tender box kept in the room of the Chairman of the Tender Board.
Bids will be opened at 2.30 p.m. on 31.03.2010 and the bidder or one of his agents can take part at this occasion.
The Procurement committee will reserve the right to accept or reject the tender in full or to accept only a part of it.
