Q- CBOC Capitated service for Gardena and East Los Angeles, California Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market conditions relative to NAICS 621498, Size standard $10.0 Million. The Network 22 Logistics Office (VISN 22) seeking potential qualified vendors to provide capitated Community Based Outpatient Clinic (CBOC) services in Gardena and East Los Angeles, California for the Greater Los Angeles Healthcare System, Los Angeles, CA. Responses to this sources sought must include the following information. 1. General Information of your company name, address, contact person name, phone number, fax number, e-mail address(s) and DUNS number and company website if available. 2. Socio-economic status of business such as 8(a) small business, Hub Zone small business, Small disadvantage business, Service Disabled Veterans owned business or Veteran owned small business. 3. Does your company have experience with Department of Veterans Affairs capitated CBOC program? 4. What does your company primarily provide? 5. Will your company enter into a joint venture or other relationship in order to provide this service? 6. How long has your company existed?
