Declaratia de participant la procedura.
Certificate de atestare fiscala privind indeplinirea obligatiilor exigibile de plata a impozitelor si taxelor de stat, inclusiv cele locale.
In cazul unei asocieri, fiecare asociat este obligat sa prezinte aceste documente.
Cerinta minima - certificat emis de Ministerul Culturii si Cultelor pentru desfasurare de activitati de executare lucrari in domeniul monumentelor istorice, aflat in termen de valabilitate si a unui extras din Registrul operatorilor economici din domeniul protejarii monumentelor istorice.

Informatii generale.
Prezentarea bilanturilor contabile aferente anilor 2007 si 2008 vizate si inregistrate de organele competente.
Cerinta minima :
-Cifra medie de afaceri pe ultimii 3 ani (2007, 2008, 2009) trebuie sa fie minim 45.000.000 lei
Se va prezenta o scrisoare bancara de confirmare a accesului la o linie de credit pentru proiectul pentru care se depune oferta, pentru o suma de minim 4.500.000 lei.
III.2.3) Technical capacity

Resurse umane:
-Efectivele medii anuale in ultimii 3 ani si resursele propuse pentru prezentul contract.
- Lista personalului responsabil - "cheie", conform fisa de date Cap.4.4.1.
Cerinta minima - Personalul propus trebuie sa aiba minim 8 ani activitate efectiva în domeniul monumentelor istorice si sa acopere cel putin functiile conform fisa de date a achizitiei Cap.4.4.1.
-Declaratie privind obligatiile referitoare la conditiile de munca si protectia muncii.
Resurse Tehnice - Declaratie privind utilajele, instalatiile, echipamentele tehnice.
Cerinta minima:
Ofertantul trebuie sa demonstreze ca detine sau ca are acces nelimitat (prin contract de închiriere sau altele similare) la:
- un laborator autorizat de analize si verificari a activitatii în constructii având minim gradul III.
- un laborator autorizat si acreditat de analize si încercari în activitatea de restaurare.

Lista principalelor lucrari din ultimii 5 ani.
Experienta similara
- Se va prezenta Fisa de informatii privind experienta similara pentru contracte similare, semnate si finalizate in decursul ultimilor 5 ani (2005, 2006, 2007, 2008, 2009).
Se solicita finalizarea cu succes, in ultimii 5 ani a unui contract de lucrari de consolidare, restaurare cladiri monumente istorice, în valoare de minim 7.000.000 lei fara TVA, de natura si complexitatea similara lucrarilor din Anexa 1 la prezenta Fisa de date, sau a maxim trei contracte de lucrari de consolidare, restaurare cladiri monumente istorice, cu o valoare cumulata a contractelor de 11.500.000 lei fara TVA, de natura si complexitatea similara lucrarilor din Anexa 1 la prezenta Fisa de date.
- certificat ISO 9001 - sistem de management al calitati pentru lucrarile acoperite de prezentul contract (sau echivalent);
- certificat ISO 14001/2005 - sistem de management de mediu pentru lucrarile acoperite de prezentul contract (sau echivalent);
- certificat ISO 18001/2004 - sistem de management al sanatatii si securitatii ocupationale pentru lucrarile acoperite de prezentul contract (sau echivalent);
Certificatele solicitate trebuie sa fie valabile la data deschiderii ofertelor.
