Declaratie privind calitatea de participant la procedura
Certificat constatator eliberat de Oficiul Registrului Comertului sau echivalent din care sa rezulte numele complet, sediul, persoanele autorizate/administratori; si codul/codurile CAEN aferente domeniului de activitate necesar pentru livrarea bunurilor si prestarea serviciilor care fac obiectul licitatiei
Certificat de atestare fiscala eliberat de organul de administrare fiscala al unitatii administrativ teritoriale de pe raza careia societatea îsi are sediul social privind plata obligatiilor la bugetul de stat sau echivalent. Certificatul trebuie sa fie valabil la data deschiderii ofertelor.
Certificat privind plata impozitelor si taxelor locale, valabil la data deschiderii ofertelor.
Cazier judiciar pentru administrator(i) asa cum sunt mentionati în Certificatul Constatator precum si pentru persoana împuternicita (unde este cazul) ce semneaza declaratia referitoare la art.180 si art.181, respectiv Declaratie privind eligibilitatea.

Se pot prezenta si orice alte acte doveditoare (certificate) echivalente în conditiile art.182 din OUG nr.34/2006 modificata si completata

Informatii generale
Bilanturile contabile aferente exercitiilor financiare 2006, 2007, 2008, vizate si înregistrate de organele competente în conditiile art. 185 din OUG nr.34/2006, modificata si completata. Pentru ofertantii nerezidenti, în cazul în care pentru exercitiul financiar 2008 bilantul si anexele nu au fost înregistrate de organele competente, ofertantul va prezenta bilantul contabil, anexele la acesta si balanta de verificare contabila intocmita la 31.12.2008, insotite de raportul auditorilor financiari si contabili autorizati, sau de cenzorii societatii dupa caz, din care sa rezulte conformitatea, sub toate aspectele semnificative, a valorilor reprezentand activele circulante, datoriile curente si veniturile din exploatare reflectate in acestea.
Media profitului net al ofertantului (operator economic sau membrii asocierii de operatori economici impreuna) pe ultimii 3 ani (2006, 2007, 2008) trebuie sa fie pozitiva.
Media Cifrei de afaceri a ofertantului (operator economic sau membrii asocierii de operatori economici împreuna) pe ultimii 3 ani (2006, 2007 si 2008) trebuie sa fie mai mare de 2.000.000 RON.
III.2.3) Technical capacity

Lider de proiect; Administrator baza de date - Consultant Senior pentru implementarea solutiei de portal, Consultant Senior pentru administrarea serverului de aplicatii, Administrator infrastructura, cu experienta mentionata in documentatia anexata prezentului anunt.
Ofertantul trebuie sa dispuna de capabilitati pentru acordarea serviciilor de helpdesk / asistenta tehnica si suport clienti.
Ofertantul va face dovada finalizarii in ultimii 3 ani a minim 1 contract de prestari servicii cu o durata de minim 10 luni, al carui obiect a constat in migrarea de date si aplicatii sau integrarea aplicatiilor si care include o aplicatie pentru business intelligence si care sa fi avut in componenta un numar minim de 5 experti cheie.
Ofertantul va face dovada finalizarii in ultimii 3 ani a minim 1 contract, de livrare si instalare de infrastructura hardware de tip RISC sau CISC si stocare, in care participatia ofertantului pe zona specifica RISC sau CISC si stocare sa fie de minim 100000 de Euro fara TVA
Ofertantul va face dovada finalizarii in ultimii 3 ani a minim 3 contracte, la care participatia cumulata a ofertantului este de minim 200000 Euro fara TVA, contracte ce au presupus livrarea de echipamente hardware, dezvoltarea de aplicatii (analiza, design, dezvoltare si integrare) si instruire
Ofertantul va face dovada finalizarii in ultimii 3 ani a minim 2 contracte pe o componeta de portal, si minim un contract care include o componeta de business inteligence.
