The G.V. (Sonny) Montgomery VA Medical Center will be soliciting proposals for primary care at a Community Based Outpatient Clinic located in Meridian, MS to provide services for VA beneficiaries. Anticipated period of performance will be April 1, 2010 to March 31, 2011 with four, one-year option periods to be renewed at the Government's discretion. Contractor shall provide medical facilities, health care providers, medical equipment, supplies and administrative support. Payment shall be made through monthly capitation rate for each patient in accordance with the terms and conditions of the resulting contract. Request for Proposal (VA-256-10-RP-0085) will be issued on or about 2/1/2009. No telephone requests will be honored. Electronic submissions (disc) are permitted. The government is not responsible to make available paper copies. Questions must be submitted in writing to the contracting officer. If contractors will submit questions, they are strongly encouraged to do so within ten (10) calendar days after issuance of the Request for Proposal (RFP). Verbal questions will not be addressed. All responses to questions which may affect offers will be incorporated into a written amendment to the solicitation. Offeror's are encouraged and are responsible for routinely checking this web-site for any solicitation amendments that may be issued throughout the proposal period. Offer due date and time for proposals as well as pre-proposal conference details, with technical documentation will be included in the solicitation document. This is an unrestricted procurement. A single award contract will be made. Contractors are required to be registered with Central Contractor Registration (CCR) before contract award. NAICS Code for this requirement is 621111. Size standard is $10.0 mil.
