Garantia de participare:174.000 lei, respectiv 41.331euro. Cuantumul garantiei de buna executie a contractului de lucrari:10% din pretul contractului, fara T.V.A.

Information and formalities for evaluating if requirements are met:
a) Situatia personala a ofertantului: 1.Declaratie privind eligibilitatea conform art. 180 din O.U.G. nr. 34/2006; 2.Declaratie privind neîncadrarea în situatiile prevazute la art. 181 din O.U.G. nr. 34/2006; 3.Certficat privind plata obligatiilor la bugetul de stat si a contributiilor la asigurarile sociale; 4.Certificat privind plata impozitelor si taxelor locale; 5.Declaratie privind calitatea de participant la procedura; 6.Imputernicire notariala privind autorizarea semnarii ofertei si contractului;7.Cazierul judiciar al societatii;8.Declaratie privind încadrarea ofertantului în categoria I.M.M.-urilor;
b) Capacitatea de exercitare a activitatii profesionale (înregistrare): 1.Certificatul de înregistrare fiscala eliberat de Oficiul Registrului Comertului; 2.Certificatul constatator eliberat de Oficiul Registrului Comertului.
c)Pentru persoane fizice române: Autorizatia de functionare.
d)Pentru persoane juridice/fizice straine:Documente edificatoare care sa dovedeasca o forma de înregistrare ca persoana juridica sau de înregistrare/atestare ori apartenenta din punct de vedere profesional, în conformitate cu prevederile legale din tara în care ofertantul este rezident. Documentele vor fi prezentate în traducere legalizata.

1.Fisa de informatii generale; 2.Bilanturi contabile din ultimii 3 ani; 3.Declaratie privind cifra de afaceri ; 4. Dovada existentei in cont a 30% din valoarea estimata a contractului sau Scrisoare de garantie bancara ;5.În cazul în care ofertantul îsi demonstreaza situatia economica si financiara invocând si sustinerea acordata de catre o alta persoana-angajament ferm al persoanei respective, încheiat în forma autentica.

Ofertanul va face dovada: 1) unei cifre medii de afaceri pentru anii 2006, 2007si 2008, de minim 20.000.000 lei respectiv, 4.750.707 euro. 2) existentei în cont a sumei 3.480.000 lei reprezentînd 30 % din vlaorea estimata a contractului sau Scrisoare de garantie bancara cu o valoare de 3.480.000 lei.

1.În cazul în care ofertantul îsi demonstreaza capacitatea tehnica si profesionala invocând si sustinerea acordata de catre o alta persoana - Angajament ferm al persoanei respective incheiat in forma autentica;2.Lista principalelor lucrari executate în ultimii 5 ani; 3.Experienta similara; 4.Informatii referitoare la personalul/organismul tehnic de specialitate; 5.Informatii referitoare la studiile, pregatirea profesionala si calificare personalului de conducere, precum si ale persoanelor responsabile pentru executia lucrarilor; 6.Declaratie referitoare la efectivul mediu anual al personalului angajat; 7.Declaratie referitoare la utilajele, instalatiile, echipamentele tehnice; 8.Licenta de functionare;9.Atestate ANRE- instalatii electr. si instalatii gaze naturale;10. Autorizatie ISCIR- centrale termice; 11. Declaratie pe propria raspundere- respectarea cond. de munca, protectia muncii; Standarde de asigurare a calitatii; 12.Standarde de protectie a mediului. 14.Informatii privind partea din contract pe care operatorul economic are intentia sa o subcontracteze.

Ofertanul va face dovada indeplinirii in ultimii 5 ani a cel mult doua contracte finalizate avand ca obiect executia de lucrari de constructii noi, cu o valoare cumulata de minim 11.600.000lei respectiv, 2.755.410 euro.
