1. - Declaratie pe proprie raspundere privind eligibilitatea - Formular B1;
2. - Declaratie privind neîncadrarea în art. 181;
3. -Certificate de atestare fiscala privind achitarea obligatiilor exigibile - la bugetul de stat, la bugetul asigurarilor sociale, taxe si impozite locale - in original sau copie legalizatã
4. - Declaratie pe propria raspundere ca nu se afla în nici una din situatiile prevazute la Art.5 alin. 1 din legea nr.21/1996 privind concurenta;
Agentii economici care intra sub incidenta legii 21/1996 si sunt implicati intr-o operatiune de concentrare economica pot participa la licitatie numai cu o singura oferta comuna, ca asociati, prezentând si acordul de asociere.
5. - Pentru persoane juridice române: - Certificat constatator in original sau copie legalizatã (valabil la data licitatiei) emis de Oficiul Registrului Comertului de pe lânga Tribunalul teritorial;
6. - Pentru persoane juridice straine: - documente edificatoare care sa dovedeasca o forma de înregistrare ca persoana juridica/atestare ori apartenenta profesionala, în conformitate cu prevederile legale din tara în care ofertantul este rezident - copie legalizatã + traducere autorizata.

1. - Lista a principalelor servicii similare prestate în ultimii 3 ani, continând valori, perioade de prestare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati;
Declaratii pe propria raspundere referitoare la :
2. - utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru îndeplinirea corespunzatoare a contractului de servicii;
3. - proportia in care contractul de servicii urmeaza sa fie indeplinit de catre subcontractanti si specializarea acestora;
4. - respectarea conditiilor de munca si protectia muncii;
5. - masurile de protectie a mediului pe care operatorul economic le poate aplica în timpul îndeplinirii contractului de servicii;
6. - Standarde de asigurare a calitatii: certificate care atesta implementarea sistemului de calitate: ISO 9001 si OHSAS 18001; certificat ISO 27001/2006 - sistem de management al securitatii informatiei; certificat SA 8000/2008 - sistem de management al responsabilitatii sociale;
autorizatie pentru detinere armament letal sau neletal;
7. - Alte autorizatii: autorizarea personalului pentru activitatea de colectare, transport, si paza valorilor banesti conform Legii 333/2003 si HG 1010/2004 inclusiv avizul Ministerului de Interne, autorizatie de la Inspectoratul de Politie judetean, in a carei raza de activitate isi are sediul societatea specializata de paza si protectie conform art. 20 - alin.9 din Legea 333/2003;
