1.Declaratie privind eligibilitatea completata în conformitate cu Formularul A din Sectiunea Formulare. - în original;
2.Declaratie privind neîncadrarea în situatiile prevazute la art. 181 completata în conformitate cu Formularul B din Sectiunea Formulare. - în original;
3.Declaratie pe propria raspundere ca nu se afla în nici una din situatiile prevazute la Art.5 alin.1 din legea nr.21/1996 privind concurenta completata în conformitate cu Formularul N din Sectiunea Formulare - în original;
Agentii economici care intra sub incidenta legii 21/1996 si sunt implicati intr-o operatiune de concentrare economica pot participa la licitatie numai cu o singura oferta comuna, ca asociati, prezentând si acordul de asociere
4.Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat-valabil la data licitatiei, în original sau copie legalizata.
5.Certificat privind plata taxelor si impozitelor locale - eliberat de Primaria locala- valabil la data licitatiei, în original sau copie legalizata.Nota:Ofertele depuse de operatorii economici care figureaza cu datorii restante neachitate catre bugetul de stat sau catre bugetul local vor fi respinse.

-declaratie pe proprie raspundere referitoare la personalul/organismul tehnic de specialitate de care dispune sau al carui angajament de participare a fost obtinut de catre ofertant, în special pentru asigurarea controlului calitatii - în original;
