3.Decl. pe propria raspundere ca nu se afla în nici una din situatiile prevazute la Art.5 alin.1 din Legea nr.21/1996 privind concurenta compl. în conf. cu Formularul N din Sect. Formulare - în original;
4.Agentii ec. care intra sub incidenta Legii 21/1996 si sunt implicati intr-o operatiune de concentrare ec. pot participa la licitatie numai cu o singura oferta comuna, ca asociati, prezentând si acordul de asociere
5.Certif. de atest. fisc. priv. achitarea oblig. exigib. catre bugetul de stat si catre bugetele asigurarilor sociale de stat, elib. de DGFPCFS -valabil, în original sau copie leg.

8.Pentru persoane juridice straine: - documente edificatoare care sa dovedeasca o forma de înregistrare ca persoana juridica/atestare ori apartenenta profesionala, în conformitate cu prevederile legale din tara în care ofertantul este rezident - copie legalizatã + traducere autorizata.

1. - Lista a principalelor lucrari similare prestate în ultimii 3 ani, continând valori, perioade de prestare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati;
Declaratii pe propria raspundere referitoare la :
2. - efectivele medii anuale ale personalului angajat si al cadrelor de conducere în ultimii 3 ani; se va nominaliza personalul responsabil de indeplinirea contractului si personalul responsabil cu asigurarea controlului calitatii;
3. - utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru îndeplinirea corespunzatoare a contractului de lucrãri;
4. - proportia in care contractul de lucrari urmeaza sa fie indeplinit de catre subcontractanti si specializarea acestora;
5. - respectarea conditiilor de munca si protectia muncii;
6. - masurile de protectie a mediului pe care operatorul economic le poate aplica în timpul îndeplinirii contractului de lucrari;
7. - 1 recomandare in original sau copie legalizata
8. - Standarde de asigurare a calitatii: certificate care atesta implementarea Sistemului de calitate: ISO 9001 sau echivalent,
9. - Se vor prezenta in copie cerificata "conform cu originalul" de catre ofertant, Autorizatie de Furnizor Feroviar insotita de Agrement Tehnic Feroviar,valabile la data licitatiei, in domeniu.
