Information and formalities for evaluating if requirements are met:
- Declaratii privind neîncadrarea în situatiile prevazute la art. 180 si 181 din OUG 34/2006
- Certificatul constatator eliberat de Oficiul Registrului Comertului care sa ateste ca ofertantul nu este în stare de faliment ori lichidare
- Certificatul constatator - privind obligatiile de plata a impozitelor, taxelor si contributiilor de asigurari sociale catre bugetele componente ale bugetului general consolidat (bugetul de stat, bugetele locale, bugetul asigurarilor sociale de stat, fonduri speciale etc.), în conformitate cu prevederile legale în vigoare în România sau în tara în care este stabilit
- Certificat de înregistrare
- Se solicita ca operatorul economic sa prezinte documente din care sa rezulte obiectul de activitate în domeniul colectarii si epurarii apelor uzate: cod CAEN 3700.

Information and formalities for evaluating if requirements are met:
1.Bilantul contabil la 31.12.2008, înregistrat de organele competente, in copie, semnata si stampilata pe fiecare pagina, cu mentiunea conform cu originalul.
2.Declaratie privind cifra de afaceri globala într-o perioada anterioara, care vizeaza activitatea din cel mult ultimii trei ani, în masura în care informatiile respective sunt disponibile (2006,2007,2008).
3. Declaratii bancare corespunzatoare pentru demonstrarea indicatorului lichiditate generala.
4. Declaratie pe proprie raspundere din care sa rezulte angajamentul ca are disponibilitatea de mijloace financiare de a sustine prestarea serviciilor pe o perioada de trei luni în situatia în care, din motive obiective, autoritatea contractanta nu poate efectua platile pentru prestatiile efectuate.

Se solicita realizarea unei cifre de afaceri medii anuale, care vizeaza activitatea din cel mult ultimii trei ani, egala sau mai mare decît valoarea minima impusa de 4.600.000 lei. Se solicita Lichiditate generala mai mica sau egala cu 100%. Se va prezenta scrisoare de bonitate bancara pentru demonstrarea indicatorilor economico financiari solicitati, emisa cu cel mult 30 de zile înainte de deschiderea ofertelor.

- Lista a principalelor servicii prestate în ultimii 3 ani, continând valori, perioade de prestare si locul prestarii serviciilor, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati.
- Declaratie care contine informatii privind asigurarea cu personal de specialitate considerat strict necesar pentru îndeplinirea contractului
- Declaratie privind echipamentele tehnice si utilajele de care dispune operatorul economic pentru îndeplinirea corespunzatoare a contractului.
Minimum level(s) of standards possibly required
Se solicita prezentarea informatiilor referitoare la servicii similare prestate în ultimii 3 ani (cod CPV 65.120.000), având o valoare cumulata, egala sau mai mare decat valoarea de 2.300.000 lei, exclusiv TVA. Se solicita asigurarea personalului de specialitate nominalizat în caietul de sarcini, respectiv: 1 sef statie, 2 electricieni, 2 lacatusi mecanici, 3 operatori, 1 laborant, 2 muncitori. Se solicita asigurarea acelor utilaje, instalatii, echipamente tehnice care sunt considerate strict necesare pentru îndeplinirea contractului, corespunzatoare tehnologiei. Se prezinta copie autorizare laborator si, dupa caz, contract de prestari servicii laborator autorizat.
