Note: Non-refundable Tender Fee :- Rs. 500/= for each Item.
Bid Bond Amount :- For Each Item 2% of Bid Value.

The Chairman, of the Ministry Procurement Committee, invites sealed bids from principals or their accredited agents for Supply, Delivery and Installation of following Medical Equipment up to 10.00 hrs of December 09th 2009.
Sealed Tender should be made in a complete set of bidding document in duplicate purchased by any interested eligible bidder, agent or nominee on behalf of the bidder on the submission of;
A) A written application to the Provincial Director of Health Services, Department of Health Services, Uva Province, Badulla.
B) Upon payment of a non-refundable fee in cash to the Accountant, Department of Health Services, Uva Province, Badulla.
Bidding document can be obtained from office of the Department of Health Services, Uva Province, Badulla from 19th November 2009 to 08th December 2009 during office hours.
Tender documents could be inspected free of charge at the office of the Department of Health Services, Uva Province, Badulla during office hours.
Sealed Bids should be sent by the registered post to the address given below or could be deposited in the tender box kept at the office before 10.00 hrs on 09th December 2009. Tenders will be opened immediately after the closing time and bidders or authorized representatives are requested to participate at this occasion.
The decision of the Ministry Procurement Committee, Department of Health Services, Uva Province, will be the final.
