SECTION II: OBJECT OF THE CONTRACT
II.1) DESCRIPTION

Evaluation of national exercise referral Scheme.

II.1.5) Short description of the contract or purchase(s): The National
Exercise Referral Scheme aims to enable health professionals to refer
patients to a qualified exercise professional for a supervised programme
of physical activity.
Earlier in 2009 the Minister for Health and Social Services gave
permission to expand the Scheme to incorporate the clients with chronic
conditions. They include clients with obesity and diabetes, coronary heart
disease (CHD), lung disease, lower back pain, who have had strokes, who
are at risk of falling, and have mental health problems.
The main aim of this contract is to examine the implementation of the
Scheme with the client with coronary heart disease, respiratory disease,
at risk of falling and with back pain.
The specific objectives are as follows:
— To evaluate the implementation of protocols and procedure developed as
part of the Scheme with clients with chronic conditions listed above,
— To identify any changes in clients’ health as part of participation in
the Scheme,
— To make recommendations to improve the implementation of the scheme with
chronic conditions.
The contract initially runs for 1 year with an option to extend for a
period up to a further 12 months, once a decision on continuation of the
scheme has been made.

reserves the right to request parent company and/or other guarantees of
