Garantie de participare 13200 lei sub forma de scrisoare de garantie emisa in conditiile legii de o societate bancara ori de o societate de asigurari sau prin virament bancar in contul ANL.

Se solicita:
- declaratie privind eligibilitatea cf. art. 180 din OUG 34/2006,
- declaratie privind neincadrarea in situatiile prevazute la art. 181 din OUG 34/2006,
- certificate fiscale care atesta indeplinirea obligatiilor de plata la bugetul local si bugetul consolidat, valabile la data deschiderii ofertelor, in original sau copie legalizata sau declaratie pe propria raspundere prin care confirma indeplinirea acestor obligatii de plata,
- certificat constatator eliberat de ORC in original sau copie legalizata,
- certificat de inregistrare fiscala eliberat de ORC in original, copie legalizata sau xerocopie certificata pentru conformitate cu originalul.

Se solicita:
- fisa de informatii generale prin care se va demonstra realizarea unei cifre medii anuale de afaceri pe ultimii 3 ani mai mare de 660000 lei,
- bilantul contabil la 30.06.2009, vizat si inregistrat de organele competente, in xerocopie certificata pentru conformitate cu originalul.

Se solicita:
- lista principalelor prestari de servicii similare cu cele ofertate in ultimii 3 ani - se va prezenta cel putin un contract al carui obiect a cuprins toate serviciile care fac obiectul prezentei proceduri de achizitie,
- documente emise de organisme acreditate care confirma certificarea sistemului calitatii, al sistemului de mediu si a sistemului de asigurare a sanatatii si securitatii muncii, respectiv ISO 9001, ISO 14001 si OHSAS 18001,
- declaratie privind efectivul mediu anual al personalului angajat si al personalului de conducere pentrui anii 2007, 2008, 2009,
- informatii referitoare la personalul tehnic de specialitate - caziere judiciare, certificate de calificare profesionala, autorizatii,
- declaratie privind dotarile tehnice specifice
- declaratie privind sanatatea si protectia muncii,
- autorizarea ISCIR INSPECT IT a societatii comerciale pentru servicii de revizie si interventii la centrala termica,
- declaratie privind subcontractantii.
