Note: Non-Refundable Tender Fee: 250/-

Applications are invited from reputed manufacturers / suppliers who wish to register themselves with the Gampaha Wickramarachchi Ayurveda Institute for the year 2010 to supply the categories of items listed below, and for various other services and construction works. A non-refundable fee of Rs. 250/ = per item should be enclosed with the applications, payment should be in cash or by Bank Draft drawn in favour of "Gampaha Wickramarachchi Ayurveda Institute".
Cash should ( be paid to ,the shroff at Gampaha Wickremarachchl Ayurveda Institute between 9.00 a.m. and 3.00 p.m. (Cheque & money orders will not be accepted).
Duly completed application forms as per specimen given below should be sent under registered cover to the Deputy Bursar, Gampaha Wickremarachchi Ayurveda Institute, Yakkala.
All applications should be marked "Registration of Suppliers and Services 2010" on the top left corner of the envelope.
Gampaha Wickramarachchi Ayurveda Institute of Sri Lanka will generally call for quotations from the registered suppliers. However, it reserves the right to call quotations from a restricted list of Suppliers or from any other suppliers as well, when deemed necessary. Supplier who fails to quote when requested or fail to make delivery in time or in conformity with the given requirements will be struck off the register without prior notice.
Closing date of applications is 03.12.2009.
