-Se vor prezenta fise referitoare la contractele de achizitie produse incheiate in ultimii 3 ani, a caror obiect a fost furnizarea de produse similare, insotita de recomandari, si care sa contina valori, perioade, modul de indeplinirea a obligatiilor, beneficiari
Ofertantul va trebui sa dovedeasca ca a incheiat si indeplinit contracte de furnizare produse similare celor supuse licitatiei, avand valori mai mari decat :
L1- 168.000 lei
L2 -26.000 lei
L3 - 250.000 lei
L4 -27.000 lei
L5 34.000 Lei
L6 -1.000 lei
Nota : Nu se vor lua in considerare contractele care nu sunt insotite de recomandari
- Se solicita obligatoriu prezentarea autorizatiei pentru livrarea produselor, acordate de producator.
(Se completeaza numai de catre furnizorii care nu sunt producatori)
- Ofertantii vor prezenta obligatoriu certficatul de asigurare a calitatii ISO 9.001 sau doc echivalent.
Ofertantii care nu sunt producatori, vor prezenta suplimentar si certificatul care atesta implementarea sistemului calitatii al producatorului care le-a acordat autorizatia de livrare.
-Ofertantul va face dovada implementarii sistemului integrat calitate mediu: ISO 14.001 sau echivalent
