Experienta similara minima impusa este de 3.700.000 lei si va putea fi dovedita prin prezentarea unor contracte de executie lucrari similare cu obiectul achizitiei, incheiate si executate in ultimii cinci ani.
Se vor prezenta procesele verbale de receptie, copie dupa contracte precum si certificarile de buna executie (recomandari ) de la beneficiari.
Se solicita prezentarea listei conform Formular 12H privind asigurarea (în dotare proprie, prin contracte sau conventii de închiriere etc.) acelor echipamente, utilaje, mijloace de transport, laboratoare, baze de productie, alte mijloace fixe si dotari care sunt considerate strict necesare pentru îndeplinirea contractului de lucrari. Dotare minima impusa (in dotare proprie sau contracte de colaborare)
- buldozer - 2 bucati
- excavator - 2 bucati
- lansatoare 4 bucati
Laborator autorizat pe metoda incercari nedistructive metale Rt si Rp;
Laborator analiza si incercari instalatii in constructii;
Se solicita prezentarea listei conform Formular 12I privind asigurarea cu personalul de specialitate care este considerat strict necesar pentru îndeplinirea contractului de lucrari, asigurarea de catre ofertant a unui responsabil tehnic cu executia care sa fie atestat în domeniul lucrarii ce urmeaza a fi executate.Informatii privind subcontractantii Formular 12 G ;Se vor prezenta recomandari din partea beneficiarilor de lucrari similare solicitate la pct V4a;
Ofertantii vor prezenta obligatoriu
-documente de certificare a sistemului calitatii ISO 9001:2001, sau echivalent, domeniul de activitate fiind similar cu obiectul achizitiei.
- documente de certificare a sistemului calitatii ISO 14001:2001, sau echivalent, domeniul de activitate fiind similar cu obiectul achizitiei;
- documente de certificare a sistemului de Management al Sanatatii si Securitatii Ocupationale OHSAS 18001, sau echivalent;
- autorizatia ETR si ETC emise de ANRE ;
Atestat tip B, emis de ANRE pentru instalatii electrice de 0,4 KV,sau echivalent.;Se va prezenta o declaratie pe propria raspundere prin care se mentioneaza respectarea conditiilor de mediu impuse de legilslatia in vigoare.
