Sealed Bids are invited by the Chairman, Ministry Procurement Committee, Ministry of Healthcare & Nutrition for the Provision of Cleaning Services for a period of one year, from 01.01.2010 up to 31.12.2010, for the provision of cleaning services for the under mentioned hospitals and institutes functioning under the Ministry of Healthcare & Nutrition, from institutes which fulfill the under mentioned conditions.
Basic conditions required:
Should be an institute registered 03 years earlier as "an institute supplying cleaning services" under the Business Names Ordinance.
Should have a minimum of 03 years' experience in the provision of Cleaning services for government institutes (specially government hospitals) and the value of a such single institute should be equal to Rs. 1,000,000/ = or more and this should be proved by letters obtained from such institutes.
Should submit Receipts and C Report obtained from the Central Bank of Sri Lanka, for the monthly payment of Employees' Provident Fund and the Employees' Trust Fund (EPF & ETF) contributions for 150 employees under the Registered Business Name, for the nearest previous year. Further, institutes employing more than 150 persons should submit certificates to substantiate the above contribution payments to its employees.
Should prove with documents the payment of VAT / Income Tax under the relevant institute's name engaged in cleaning services, for the nearest previous 02 years. Limited companies in the relevant company's name, individuals / partnership companies in the name of the owner or names of main partners should have paid the income tex, VAT, and other taxes (Economic Service Tax, Nation Building Tax, etc), in their names and should submit documents accordingly.
Bid applications can be obtained from 12.11.2009 up to 03.12.2009 from 9.00 a.m. & 2.00 p.m. during working days of the week, from the Procurement Section of the Ministry of Healthcare & Nutrition at No. 385, "Suwasiripaya", Ministry of Healthcare & Nutrition, Ven. Baddegama Wimalawansa Thero Mawatha, Colombo 10. The receipt for the payment of Rs. 2500/= non-refundable fee to the Accountant, by cash, along with the documents under mentioned, should be submitted when obtaining the bid documents.
(i) Business Registration Certificate.
(ii) Worth certificate for Rs. 500,000/= obtained from the Divisional Secretary within 06 months, on Form General 170.
(iii) Experience Certificates for 03 years.
Completed bid documents should either be deposited in the tender box kept at the office of the Assistant Secretary (Procurement) or sent by Registered Post addressed to Chairman, Ministry Procurement Committee C, Ministry of Healthcare & Nutrition at No. 385, "Suwasiripaya", Ven. Baddegama Wimalawansa Thero Mawatha, Colombo 10, to be received at or before 10.00 a.m. on 04.12.2009. The cover containing the tender should be marked "Supply of Cleaning Services for Hospital / Institute" on the left side top corner.
