Note: Non-refundable Fee of Rs. 250/= Per Application.

Applications will be received by me up to 2.00 p.m. on 15th December 2009 from manufacturers, agents, private business establishments and from other eligible suppliers for the following supplies and services.
Applicants who seek registration for more than one item, should submit a separate application for each item. Application should be submitted accompanied with the receipt issued by the Shroff, Financial Management Unit, Chief Secretary's Office, Kurunegala, for a non-refundable fee of Rs. 250/= per application. If it is not possible to come to the Chief Secretary's Office and pay nonrefundable fee, the applicant may submit the application with a money order written in favour of "Chief Secretary, North-Western Provincial Council". (Cheques will not be accepted.) Specific application form for this purpose will not be issued and applicants may prepare own application including the following particulars.
1) Supplier's Name and Address.
2) Telephone No. / Fax No.
3) Item No. of the Supply / Service applied for registration.
4) Certificate No. of the Business Registration (A copy should be attached).
5) Ability to transport goods purchased.
The supplies and services should be undertaken on 30 days' credit basis. Quotations will be called only from the registered list of applicants but the Chief Secretary reserves the right to call for quotations from other suppliers when competitive bidding is the most appropriate. The Chief Secretary also reserves the right to reject any application sent for registration.
Suppliers who fail to submit quotations or fail to effect delivery of goods and services on the due dates will be deleted from the register.
Applications should be sent by registered post to reach the Chief Secretary, North-Western Province, Chief Secretary's Office, Kurunegala before 2.00 p.m. on 15th December 2009. "Registration of Suppliers - 2010" should be written on the left-hand corner of the envelope.
