Note: Each application should accompany a receipt to the value of Rs. 750/= which is a Non-refundable fee for the registration in favour of Colombo South Teaching Hospital. Cheques and money orders will not be accepted.

Applications are invited from Manufacturers, Suppliers, Sole Agents, Registered Co-operative Societies, Gramodaya, Registered Business Organizations, Individuals, who wish to register themselves with this institution for the supply of items and services listed below for the year 2010.
Conditions :-
Separate applications should be forwarded for each of the above supplies/services.
Duly completed application forms as per specimen given below with a copy of business registration should be sent on or before 30th November 2009 under registered cover to the Director, Colombo South Teaching Hospital, Kalubowila, Dehiwala or should be placed in the Tender Box of the Accountant Office. All application envelopes should be marked "Registration for Suppliers and Services 2010" on the top.
The Colombo South Teaching Hospital will generally call for quotations from the registered suppliers. However it reserves the right to call quotations from a registered list of suppliers or from any other supplier as well when deemed necessary. Suppliers who fail to quote when requested or fail to make supplies inconformity with the given requirements will be struck off the register without prior notice.
