-Declaratie privind eligibilitatea-original
-Declaratie privind neîncadrarea la art. 181-original
-Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat (sanatate,pensii,somaj), eliberat de Directia Generala a Finantelor Publice si Controlul Finantelor de Stat, in termenul de valabilitate prevazut in procedura fiscala-original
-Certificat privind plata taxelor si impozitelor locale-eliberat de Primaria locala, in termenul de valabilitate prevazut in procedura fiscala-original.
Ofertele depuse de operatorii economici care figureaza cu datorii restante neachitate catre bugetul de stat sau catre bugetul local vor fi respinse.
-Declaratie pe propria raspundere ca nu se afla în nici una din situatiile prevazute la Art.5 alin. 1 din legea nr.21/1996 privind concurenta-original
-Declaratie privind calitatea de participant la procedura-original
-Certificat constatator emis cu cel mult 30 de zile înainte de data deschiderii ofertelor, de Oficiul Registrului Comertului-original

-Bilantul contabil la 31.12.2008 , vizat si înregistrat de organele competente - în copie cu mentiunea "conform cu originalul
-Declaratie privind cifra de afaceri pe ultimii 3 ani, care trebuie sa fie de doua ori valoarea maxima estimata a unui contract subsecvent(care este de 628.000lei fara TVA) -original

Information and formalities for evaluating if requirements are met:
-Lista cu principalele livrari de servicii similare cu serviciile ofertate, efectuate în ultimii 3 ani, a caror valoare totala trebue sa fie mai mare decâ valoarea maxima estimata a unui contract subsecvent (care este de 628.000lei fara TVA)- original.
Se vor anexa un numar de minim 3 recomandari, scrise si parafate, din partea beneficiarilor regasiti in lista, privind calitatea serviciilor similare livrate- original.
-Aviz de functionare eliberat de Ministerul Sanatatii - în copie
-Declaratie pe proprie raspundere referitoare la echipamentele tehnice care vor fi folosite de ofertant pentru asigurarea executarii serviciilor si la masurile aplicate în vederea asigurarii calitatii - original
- Declaratie pe proprie raspundere referitoare la personalul tehnic de specialitate, care va fi folosit de ofertant pentru asigurarea executarii serviciilor si controlului calitatii acestora - original
-Acord de asociere, daca este cazul - original
- Certificat privind asigurarea de catre ofertant a Sistemului de managment al calitatii - copie legalizata. Nu se accepta societati in curs de certificare.
