declaratii de eligibilitate conform art. 180 si 181; certificat constatator emis de ORC in original sau copie legalizata; copie dupa statutul societatii; copie dupa certificatul de inregistrare emis de ORC;certificate de atestare fiscala pentru plata somaj, asigurari sociale si de sanatate; certificat de plata a impozitelor si taxelor locale;

bilant contabil si cont de profit si pierdere la 30.06.2009 vizate si inregistrate, rapoarte anuale ale administratorilor dupa caz; fisa de informatii generale; cifra medie de afaceri pe ultimii 3 ani sa fie egala sau mai mare de 5.000.000 lei; lichiditatea generala sa fie peste 100%;

- O lista a lucrarilor executate în ultimii 5 ani (minim 2 contracte similare) însotite de certificari de buna executie/recomandari pentru cele mai importante lucrari si care vor contine valori, perioada si locul executiei lucrarilor, modul de îndeplinire a obligatiilor, beneficiari;
Informatii referitoare la personalul/organismul tehnic de specialitate de care dispune, în special responsabilul pentru executia lucrarilor si pentru asigurarea controlului calitatii; informatii referitoare la studiile, pregatirea profesionala si calificarea personalului de conducere, precum si ale persoanelor responsabile pentru executia lucrarilor; o declaratie referitoare la efectivele medii anuale ale personalului angajat si al cadrelor de conducere în ultimii 3 ani; declaratie referitoare la utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru indeplinirea corespunzatoare a contractului de lucrari; recomandari din partea altor beneficiari/clienti - minim 3 recomandari; informatii privind proportia in care contractul de servicii urmeaza sa fie indeplinit de catre subcontractanti si specializarea acestora;
