1.Decl priv eligib compl în conform cu Form A din Sect Form - în orig;2.Decl priv neîncadrarea în sit prev la art. 181 compl în conform cu Form B din Sect Form.-în orig;3.Certif de atestare fiscala priv achitarea obligat exigibile catre bugetul de stat si catre bugetele asig. sociale de stat (sanatate, pensii, somaj), eliberat de DGFPCFS, val la data deschid of - în orig sau copie legaliz.4.Certif priv plata taxelor si impozitelor locale, elib de Primaria locala, val la data deschid of - în orig sau copie legaliz 5.Pt persoane juridice române:Certif constat, în orig sau copie legaliz, emis, cu cel mult 30 de zile înainte de data deschiderii ofertelor de ORC de pe lânga Tribunalul teritorial, din care sa rezulte ca:- dom de activitate al ofert corespunde obiectului procedurii;- nu sunt înscrise mentiuni cu privire la aplicarea Legii nr. 85/2006 privind procedura insolventei, sau ca societatea se afla în incapacitate de plata.6.Pt pers fiz române:- autoriz de functi, precum si orice alt document edificator considerat necesar pentru dovedirea apartenentei la categoria profesionala impusa de îndeplinirea contractului - în copie legaliz;7.Pt pers fizice/juridice straine:- doc edificatoare care sa dovedeasca o forma de înregistrare ca pers jur sau de înregistrare/atestare ori apartenenta profesionala, în conformitate cu prevederile legale din tara în care ofertantul este rezident. (doc se va prez în copie legaliz, anexându-se si traduc autoriz în lb româna)

Information and formalities for evaluating if requirements are met:
a)lista a principalelor livrari de produse similare efectuate în ultimii 3 ani, continând valori, perioade de livrare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati. Se va completa si depune, în original, Formularul C1 din Sectiunea Formulare.
Livrarile de produse se confirma prin prezentarea unor certificate/documente emise sau contrasemnate
de o autoritate ori de catre clientul beneficiar;
b)certificate sau alte documente emise de organisme abilitate în acest sens, care sa ateste conformitatea produselor - certificate de calitate / conformitate pentru fiecare tip de produs ofertat.Produsele vor fi insotite si la livrare de Certificat de calitate si Declaratia de conformitate eliberata de furnizor sau producator;
c)mostra pentru fiecare produs ofertat.
d)informatii privind partea din contract pe care operatorul economic are, eventual, intentia sã o subcontracteze - nu se prezinta daca ofertantul nu subcontracteaza.
e)Standarde de asigurare a calitatii:Se vor prezenta, în copie certificate care atesta implementarea sistemului de calitate ISO 9001 sau documente edificatoare ca se afla in curs de implementare
