III.1) CONDITIONS RELATING TO THE CONTRACT
III.1.1) Deposits and guarantees required
Garantie de participare in valoare de 354.356 RON pentru etapa a II-a a procedurii(depunerea ofertelor) si garantie de buna executie in valoare de 10% din pretul contractului, excl.TVA

Bugetul de stat si credite externe, conform HG nr 572 din 26 Aprilie 2006.

Contractul se incheie cu clauza suspensiva. Contravaloarea serviciilor prestate si a produselor livrate in anii 2010 si 2011 se va achita in conditiile existentei in bugetul ANPM a alocarilor bugetare anuale corespunzatoare.

1. Declaratie pe propria raspundere privind eligibilitatea - Formularul nr. 1
2. Declaratie privind neîncadrarea în prevederile art. 181 - Formularul nr. 2
3. Certificate constatatoare privind indeplinirea obligatiilor de plata taxelor, impozitelor si altor creante bugetare emise de catre ANAF, valabile la data limita de depunere a ofertei.
4. Certificat constatator privind îndeplinirea obligatiilor de plata taxelor, impozitelor si altor creante bugetare emise de catre autoritatea administratiei publice locale competenta, valabil la data limita de depunere a ofertei.

Pct. 1. Se va completa Formularul nr. 3 Pct. 2. Media cifrei de afaceri a ofertantului pe ultimii trei ani (2006, 2007, 2008) trebuie sa fie egala sau mai mare cu suma de 105.000.000 RON.

- Prezentarea informatiilor prin care se confirma furnizarea/prestarea de catre operatorul economic de produse/servicii similare
- Prezentarea informatiilor referitoare la organismul tehnic de specialitate de care dispune cadidatul, în special pentru asigurarea calitatii din punct de vedere al competentelor pentru realizarea integrarilor necesare Sistemului Integrat de Mediu
- Formularul nr. 5 - Lista cu personalul de specialitate
- Formularul nr. 7 - Autorizare producator pentru livrarea produselor COTS (Commercial off-the-shelf")
- Informatii privind asocierea

Nivel minim: Operatorii economici vor trebui sa prezinte: cel putin un contract de implementare/ furnizare a unui sistem informatic integrat in domeniul mediului, finalizat cu succes in ultimii 3 ani la nivel national; cel putin 2 contracte de implementare/furnizare de sisteme informatice integrate, la nivel national /regional; cel putin un contract de furnizare/implementare de sistem integrat de monitorizare finalizat cu succes in ultimii 3 ani; cel putin un contract furnizarea/realizarea unui sistem informatic, la nivel national , finalizat cu succes in ultimii 3 ani, care sa fi presupus prestarea de servicii de analiza, dezvoltare, testare si implementare software, care sa fi inclus si cel putin o componenta de monitorizare si auditare a bazelor de date; cel putin un contract de prestari servicii de asistenta tehnica si realizare a unui sistem informatic, finalizat cu succes in ultimii 3 ani, al carui scop sa fie culegerea si centralizarea datelor la nivel national precum si prezentarea acestor date sub forma de aplicatii web gis in cadrul unui portal informational cu caracter public; competente dovedite prin certificarile urmatoare (sau echivalent): business intelligence, soa and business process, independent software vendor, custom web development, advanced infrastructure, parteneriat dezvoltare gis;formularul nr. 5 va fi însotit de cv-ul în format europass, aferent fiecarei persoane propusa în cadrul echipei de proiect. personal specializat dupa cum urmeaza: director de proiect-1 specialist, project manager-1 specialist, coordonator tehnic pentru supervizarea ciclului de viata al proiectului-1 specialist, expert in sisteme de asigurarea calitatii si securitatii informatiei-1 specialist, expert arhitect in domeniul aplicatiilor enterprise-1 specialist, expert coordonator gis in domeniul mediului-1 specialist, expert implementare procese de business si integrare-1 specialist, expert instructor tehnologic-2 specialisti, expert mesagerie electronica si integrare-2 specialisti, expert în instruire utilizând medii virtuale-1 specialist, expert infrastructura hardware-1 specialist, experti in testarea sistemelor integrate-3 specialisti.
