3. Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat (sanatate, pensii, somaj), eliberat de Directia Generala a Finantelor Publice si Controlul Finantelor de Stat.
4. Certificat privind plata taxelor si impozitelor locale-eliberat de Primaria locala.
5. Certificat constatator, emis de Oficiul Registrul Comertului de pe langa Tribunalul teritorial.

III.2.3) Technical capacity
Information and formalities for evaluating if requirements are met:
1. Lista a principalelor lucrari similare executate in ultimii 5 ani (lista va cuprinde valori, perioada si locul executiei lucrarilor, modul de indeplinire a obligatiilor, beneficiari, indiferent daca acestia sunt autoritati contractante sau clienti privati).Prin informatiile furnizate, ofertantul trebuie sa faca dovada incheierii si indeplinirii in ultimii 5 ani a cel putin trei contracte al caror obiect a fost executia de lucrari similare si care sa fi avut o valoare insumata egala sau mai mare decat 90.000 lei, echivalentul a 20.930 euro.Se vor anexa in copie contractele la care se face referire si recomandarile aferente emise de beneficiari.
2. Declaratie pe proprie raspundere referitoare la personalul/organismul tehnic de specialitate de care dispune sau al carui angajament de participare a fost obtinut de catre ofertant, in special pentru asigurarea controlului calitatii, nominalizand persoana responsabila cu asigurarea calitatii.
3. Declaratie pe propria raspundere referitoare la studiile, pregatirea profesionala si calificarea personalului de conducere, precum si ale persoanelor responsabile pentru executia lucrarilor.
4. Declaratie pe propria raspundere referitoare la efectivele medii anuale ale personalului angajat si al cadrelor de conducere in ultimii 3 ani.
5. Declaratie pe propria raspundere privind masurile de protectie a mediului pe care operatorul economic le poate aplica in timpul indeplinirii contractului de lucrari.
6. Declaratie pe propria raspundere referitoare la utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru indeplinirea corespunzatoare a contractului de lucrari.
7. Declaratie pe propria raspundere privind respectarea timpilor de interventie / constatare / reparare.
8. Declaratie pe propria raspundere privind proportia in care contractul de lucrari urmeaza sa fie indeplinit de catre subcontractanti si specializarea acestora.
