This is a combined synopsis/solicitatioon for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued using simplified acquisition procedures 13.5. This solicitation is a Request for Quotation (RQ) seeking a contractor capable of developing or modifying an existing program, planning, implementing, and providing measures to evaluate the cognitive-behavioral restructing, family support, and aftercare groups. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition circular 2005-34. The associated North American Industrial Classification System (NAICS) code for this procurement is 624190, Other Individual & Family Services. Offerors must submit their quote in response to solicitation CSOSA-10-Q-0003 prior to November 30, 2009 at 11:00 A.M. EST (local). Electronic versions of the offeror's quote will not be accepted. Offerors may submit their quote via overnight express, by courier, in person delivery or by U.S. Postal Services. Proposals received after the above date and time will be considered late and will not be considered for award. See attached Document for Cover Letter, SF 18 SOW and Evaluation Criteria.
