- Experienta similara va putea fi dovedita prin prezentarea unor contracte de executie lucrari similare cu obiectul achizitiei, incheiate si executate in ultimii cinci ani.
Se solicita prezentarea listei conform Formular 12H privind asigurarea (în dotare proprie, prin contracte sau conventii de închiriere etc.) acelor echipamente, utilaje, mijloace de transport, laboratoare, baze de productie, alte mijloace fixe si dotari care sunt considerate strict necesare pentru îndeplinirea contractului de lucrari. -Dotare minima impusa:
- Utilaje de interventie rapida (trailere - 3 bucati,lansatoare - 6 bucati,macarale cu senile -2 bucati ,excavatoare - 4 bucati, buldoexcavatoare - 3 buc, buldozere - 2 bucati , surse de sudare automata -2 bucati, in dotare proprie/contract de colaborare
- Laborator grad I si grad II in dotare proprie /contract de colaborare si calificare pentru izotest -tranzotest;
Laboratoare pentru analiza defectoscopica nedistructiva,suduri,incercari mecanice si laborator de betoane in dotare proprie/contract de colaborare;
Se solicita prezentarea listei conform Formular 12 I privind asigurarea cu personalul de specialitate care este considerta strict necesar pentru indeplinirea contractului de lucrari.
Cerinta minima:
-minim 10 sudori autorizati ISCIR ,API 1104;se va prezenta copie dupa autorizatie, contract de munca permanent /colaborare.
-1 responsabil tehnic cu executia;
- 1 instalator autorizat grad I T ;
- 1 instalator autorizat grad II T
-1 instalator autorizat grad III T.
Informatii privind subcontractantii;Se vor prezenta recomandari din partea beneficiarilor ;
- Ofertantii vor prezenta obligatoriu
-documente de certificare a sistemului calitatii ISO 9001:2001, sau echivalent, domeniul de activitate fiind similar cu obiectul achizitiei.
- documente de certificare a sistemului calitatii ISO 14001:2001, sau echivalent, domeniul de activitate fiind similar cu obiectul achizitiei;
- documente de certificare a sistemului de Management al Sanatatii si Securitatii Ocupationale OHSAS 18001, sau echivalent.
Opertatorii economici vor prezenta in mod obligatoriu autorizatiile ETR si ETC emise de ANRE;
