Invitation for Bids (IFB)
Health Nutrition and Population Sector Programme(HNPSP) Credit No. 4052-BD

IFB No. CMSD/G-820/ICB/2008-2009/D-4/17 Date: 28/10/2009

1. This Invitation for Bids follows the General Procurement Notice for this Project that appeared in Development Business (UNDB) issue no. 652 of April 16,2005.

2. The “ Government of the People’s Republic of Bangladesh” has received a credit from the International Development Association toward the cost of Health, Nutrition and Population Sector Program and it intends to apply part of the proceeds of this credit to payments under the Contract for procurement of Dental Chair with Accessories.

3. The Central Medical Stores Depot now invites sealed bids from eligible and qualified bidders for supply : Dental Chair with Accessories.
Lot 1: Chair mounted Dental Unit with all accessories-31 Nos., Dental Chair with muster unit set-30 Nos., Halogen light photocuring machine-5 Nos., Ultrasonic scaler- 5 Nos.
Lot 2: Agar gel Machine-1 No., Casting Machine Electrical -1 No., Digital burn out furnace-1 No., Digital porcellain furnace-2 Nos., Electro polishing Unit-2 Nos., Mini Gas torch for soldering-10 Nos., Mini Vibrator-5 Nos., Model trimmer-2 Nos., Preheating furnace -1 No., Sand Blaster-1 No., Spot Holder-2 Nos., Vacum Mixture-1 No.
Lot 3: Bristow’s Elevator-1 No., Cheek retractor-2 Nos., Dissecting Forceps – 143 Nos., Superior Ramus Separator- 1 No., TMJ Retractor-2 Nos.

4. Bidding will be conducted through the International Competitive Bidding (ICB) procedures specified in the World Bank’s Guidelines: Procurement under IBRD Loans and IDA Credits, May 2004, and is open to all bidders from Eligible Source Countries as defined in the Guidelines.

5. Interested eligible bidders may obtain further information from the office of the Director,( Stores & Supplies) and Line Director (Procurement, Logistics and Supplies Management),Central Medical Stores Depot(CMSD), 30, Shaheed Tajuddin Ahmed Sharani, Tejgaon,Dhaka-1208 & inspect the Bidding Documents at the address given below from 09-00 hours to 17-00 hours.
6. Qualifications requirements:
(i) The Bidder shall furnish documentary evidence that it meets the financial capability to perform the contract, if awarded.
(ii) If the bidder is Manufacturer, or agent for manufacturer of the goods then the bidder or principal respectively, should be a manufacturer of the goods for at least 3 years.
(iii) If bidder is supplier of the goods then it should have at least 2 years experience of trading in similar goods and at the same times the respective manufacturer(s) of the offered goods must fulfill the requirement of clause 6(ii).
(iv) Valid CE or US-FDA certificate of the offered model must be submitted with the offer. Purchaser reserves the right to obtain confirmation of authenticity and validity from issuer/issuing authority. Self-declaration certificate is not acceptable. Original catalogue and service manual must be submitted.
(v) Critical spare parts list with estimated/projected cost must be submitted.
(vi) Maintenance cost for 10 (ten) years must be submitted. Maintenance cost can not exceed 1% of the unit price.
(vii) Original Manufacturer’s Authorization Certificate must be submitted if the bidder is not manufacturer of the offered items.
(viii) Additional details are provided in the bidding document.

7. A complete set of Bidding Documents in English may be purchased by interested bidders on the submission of a written Application to the address below and upon payment of a non-refundable fee of BDT 1,000.00 (Bangladesh Taka One thousand) or USD 20.00 (US Dollar Twenty). The method of payment will be either in Cashier’s Cheque or in Certified Cheque or in Pay Order or Bank Draft from any scheduled Bank in Bangladesh. Bidding documents will be delivered from the counter of CMSD to the bidders or their local agent.

8. Bids must be delivered to the address below at or before 11-00 AM on 21/12/2009 Electronic bidding will not be permitted. Late bids will be rejected. Bids will be opened in the presence of the bidders authorized representatives who choose to attend in person at the address below at 11-30 hours on 21/12/2009. All bids must be accompanied by Bid Security of:

The amount of the Bid Security shall be:
Lot –1: BDT 250,000.00 (BDT Two hundred fifty thousand) or US$ 3,500.00 (US Dollar Three thousand five hundred).
Lot – 2 : BDT 50,000.00 (BDT Fifty thousand) or US$ 700.00 (US Dollar Seven hundred).
Lot – 3 : BDT 200,000.00 (BDT Two hundred thousand) or US$ 2,800.00 (US Dollar Two thousand eight hundred) or equivalent amount in a freely convertible currency through a schedule bank in Bangladesh.

9. A pre-bid conference will be held on 18/11/2009 in the office of the undersigned at 11-30 AM. All Bidders are requested to send one competent authorized representative to attend the conference in time
