3. Certificat constatator eliberat de Ministerul Justitiei - Oficiul Registrului Comertului sau echivalent din care sa rezulte nume complet, sediul, persoanele autorizate/administratori;
4. Adresa de la Curtea Districtuala / Instanta Competenta / Agentie Guvernamentale unde îsi are sediul compania / sucursala sau echivalent din care sa rezule ca operatorul economic nu este in insolventa/faliment/aranjament cu creditorii;
5. Certificat de atestare fiscala eliberat de organul de administrare fiscala al unitatii administrativ teritoriale pe raza careia societatea îsi are sediul social privind plata obligatiilor la bugetul de stat sau echivalent;
6. Certificat privind plata impozitelor si taxelor locale;
7. Certificat pentru plata CAS;
8. Cazier judiciar pentru administratorul/administratorii asa cum sunt mentionati in Certificatul Constatator care semneaza "declaratia privind eligibilitatea". În cazul în care declaratia mentionata este semnata de o persoana împuternicita, se va prezenta cazierul judiciar pentru aceasta persoana precum si cel al administratorului/administratorilor care a/au împuternicit aceasta persoana.

2.2.1. Media profitului net al ofertantului (operator economic sau membrii asocierii de operatori economici impreuna) pe ultimii trei ani (2006, 2007, 2008) trebuie sa fie pozitiva. 2.2.2. Media Cifrei de afaceri a ofertantului (operator economic sau membrii asocierii de operatori economici impreuna) pe ultimii trei ani (2006, 2007, 2008) trebuie sa fie mai mare de 1.000.000 euro.

1.Informatii referitoare la personalul tehnic de specialitate de care dispune ofertantul (sau al carui angajament de participare a fost obtinut).
2.Lista principalelor prestari de servicii în ultimii 3 ani.

1.Ofertantul sa dispuna de cel putin 10 (zece) specialisti (personal tehnic de specialitate) care sa lucreze in domeniul comunicatiilor si de care sa dispuna la implementarea contractului pe toata perioada de derulare a acestuia. Nivelul minim de pregatire al specialistilor pentru echipamentele de comunicatii de date implicati in implementarea proiectului va fi de: - 2 specialisti cu certificare CCIE - 3 specialisti cu specializari in domeniile CCNP si CCNA, - 1 specialist JNCIA - 1 specialist JNCIS - 1 specialist JNCIE - 1 expert care sa detina certificarile: • Project Manager - Prince 2 • Lead Auditor ISO 27001 • CompTIA Security+; - 1 expert care sa detina certificarile: • CISM • CGEIT sau certificate echivalente. 2.Ofertantul a finalizat maxim 3 contracte la nivel national ce au avut ca obiect prestarea de servicii transmisii date de tip VPN, în ultimii trei ani împliniti la data termenului limita pentru depunerea ofertei; Participarea cumulata a ofertantului la aceste contracte trebuie sa fi fost de minim 1.000.000 euro. 3.a)Ofertantul trebuie sa aiba implementat un sistem de management al calitatii respectiv ISO 9001 sau echivalent, b)Ofertantul trebuie sa aiba implementat un sistem de management al securitatii informatiilor ISO 27001(sau echivalent) c) Ofertantul trebuie sa aiba implementat un sistem de management de mediu ISO 14001 sau echivalent. d) Ofertantul trebuie sa aiba implementat un sistem de management al sanatatii si securitatii ocupationale OHSAS 18001 sau echivalent. 4.Ofertantul trebuie sa detina: -Centru de training oficial - autorizat de producatorul echipamentelor de comunicatii la nivel LAN in Romania -Material oficiale de curs de la producatorul echipamentelor de comunicatii la nivel LAN -Instructor (trainer) autorizat de producatorul echipamentelor de comunicatii la nivel LAN in Romania - vorbitor de limba romana -Cursul trebuie predat in limba romana
