The Department of Veterans Affairs, VISN 11 intends to solicit offers from SMALL BUSINESS firms only who are interested in providing off-site optical dispensing, patient education, eyeglass sales, repairs and related services for eligible veterans for the VA Community Based Outpatient Clinic, (CBOC or VA care site), at 411 DR Martin Luther King Dr., Peoria, Illinois, 61605. The contract period will be one (1) year from contract award, with up to four (4) one year renewal options if exercised. The NAICS code is 541990 and size standard is $6M. This solicitation is 100% set aside for small business as a commercial item in accordance with FAR Part 12 and FAR Part 13.
