Note: A fee of Rs. 842.50 including tax per items will be levied for separate registration of each supply and services above.

Applications are invited for supply of following goods and services for the year 2010 from reputed Manufacturers, Suppliers / Contractors.
Applications may be sent either by registered post or hand delivered to "Municipal Commissioner, Municipal Council, Badulla" on or before 16.11.2009. "Registration of Suppliers / Contractors and Organizations Supplying Security Services - 2010" should be mentioned on the top left corner of the envelope containing the applications. Registration fee for each supply and service with the taxes 12% VAT + 1% NBT could be paid by cash or in the form of money orders or bank drafts in the name of "Municipal Commissioner, Municipal Council - Badulla". The amount payable has been mentioned at the conclusion of this notice.
Applications for registration as contractors and for supply of security services should be made only through the form obtainable from this institute. Relevant applications will only be issued up to 2.30 p.m. on 13.11.2009. Application requesting for forms should be sent together with Name, Address, Telephone Number, Copies of Business Registration Certificate, ICTAD Registration Certificate and Local Government Bodies Registration Certificate.
Self-prepared applications for supplies and services should be sent together with copies of relevant certificates. (A copy of Business Registration Certificate and if within the city limits of Badulla, a copy of receipt for payment of trading licence for the year 2009 should be submitted).
Conditions for registration of suppliers / contractors / for supply of security services :- The Council has the right to purchase goods by calling fresh quotations when required, besides the registered suppliers.
The registration of suppliers who fail to submit samples and specimens when required and fail to supply goods of high quality with specified standard at specified time as per orders, will be cancelled without intimation.
Suppliers should provide credit facilities for minimum of 60 days from the date of supply of goods.
Advance payment will not be made for the customers.
All supplies and services are subject to taxes and stamp duties imposed by the government.
Requests of suppliers who failed to complete the orders in the past years and whose names have been struck off from the register, will be rejected.
Certified copies of relevant certificates / documents to prove the facts in the application form should be attached to the application, when applying for security service.
Should be able to produce the original copies of certificates and documents requested, for scrutinizing whenever required.
The applicant should produce the registration book of ICTAD Institution and should have the valid registration for the year 2010 when applying to register as contractors to do the civil works of Municipal Council Badulla.
Perfected applications together with a copy of Registration Certificate of ICTAD, copy of statements of assets, a copy of VAT registration certificate and copies of other relevant documents should be submitted.
Application submitted late will be rejected.
