Applications are invited from Sri Lankans / Registered Trade Companies / Cooperative Societies / Manufacturing Institutions who wish to register themselves for the supply of Goods and Services to the Teaching Hospital of Kurunegala for the year 2010.
02. The persons or institutes, that wish to register as suppliers-should direct their applications including the details according to the under mentioned 6th paragraph, quoting the item number and the item or items, you wish to supply. Applications should be sent separately for each item with a receipt, obtained by paying a non-refundable Rs 750/ = to Shroff of Hospital Cheque and Money Orders are not acceptable
Conditions: The applicants should mention specially in their applications about the ability in providing a particular service, the extent of providing transport facility and Ability to provide on credit facilities and if the applicant;; has any special conditions.
All applicants should send their applications on 16th December 2009 at 11.00 a.m. to Chairman, Divisional; Procurement Committee, Teaching Hospital, Kurunegala by Registered Post. You have to write "Registration of Service Providers for 2010" on the upper left-hand corner;; of the envelope or deposited in the Tender Box available at;; the Accountant's Office.
Registration fee will not be refunded, If application is ; rejected.
Usually the quotations are taken from registered service , providers and contractors. But the institution has the right of taking quotations and the right to purchase and take services from any other service provider or contractor.
Those not giving quotation when requested, not providing items in time when it is ordered, and those not providing the items according to the given samples will be erased from the registration list without prior notice. It is necessary to include following details in all' applications.
The addresses, Telephone numbers, Fax numbers and VAT Numbers of the contractors and service provider; The sort of services and items to be provided; The name and the registration number of the Business; Nature of the provider (Sales representative) distributor & product wholesale; Where the services provided on Credit basis Details of the duration and value); The amount of the money paid and the receipt number when payment is made.
