SECTION II: OBJECT OF THE CONTRACT
II.1) DESCRIPTION
II.1.1) Title attributed to the contract by the contracting authority:
Chronic Disease Self-Management Programme.

II.1.5) Short description of the contract or purchase(s): A consortium of
West Midlands Primary Care Trusts would like to invite applications from
suitably-qualified providers for the provision of Chronic Disease
Self-Management (CDSM) programmes within the following PCT areas:
— Coventry,
— Shropshire,
— South Staffordshire,
— Stoke-on-Trent,
— Telford & Wrekin and,
— Walsall.
Providers can submit an application to provide the services within all or
some of these PCT areas.
The West Midlands SHA is administering this procurement process on behalf
of the consortium members and is not procuring the services for itself.
The remaining eleven (11) West Midlands Primary Care Trusts may
participate in the procurement at some later point.
The Chronic Disease Self-Management Programme (often referred to as the
expert patient programme) is a self-management educational programme for
individuals with a long term condition. Developed by Stanford University,
the programme aims to equip participants with the confidence, skills and
motivation to better manage their condition, work more effectively with
social and health care professionals, and take control of their health,
thus improving their quality of life.
The consortium wishes to engage with as many providers of services as
possible, so contracts will be awarded to all providers who pass the
accreditation process. Providers must hold a licence from Stanford
University in order to the deliver the programme. Providers will be free
to recruit service users from the PCT areas where they are accredited to
carry out the services. It will be the responsibility of providers to seek
out service users.
Payment under the contract is based on a local tariff devised by the
consortium as follows:
Participants completing the programme.
General Population - GBP313.59.
Targeted Populations - GBP402.49.
Note: targeted populations are people from ethnic minorities, residents of
deprived areas, persons aged 25 years and under, people requiring
assistance accessing the course materials (e.g. people with visual or
hearing impairment).
While it is not possible to provide an accurate estimate of the potential
value of the contracts that will be awarded in connection with CDSM
programmes, the latest data suggests that there are approximately 950 000
residents in the West Midlands with chronic long-term health conditions of
which 330 000 are resident in the participating six PCT areas. The
consortium spent approximately GBP 180 000 on CDSM programmes in 2007/08.
Over the period of the contract, the consortium intends to substantially
increase the level of service provision, giving an estimated total
contract value in the region of GBP 200 000 – GBP 500 000 per annum,
depending on take-up of the service.
Note: At this stage the other eleven West Midlands PCTs (North
Staffordshire, Warwickshire, Herefordshire, Dudley, Wolverhampton,
Solihull, Heart of Birmingham, South Birmingham, Birmingham East and
North, Sandwell and Worcestershire) are not participating in this
procurement. Should they decide to participate, the value and scope of the
contracts for services will increase accordingly and the estimated spend
of these PCTs will be included in the contract notice which advertises
their inclusion in the consortium.
Contracts will be awarded for the provision of the services from 1.4.2010.
Contracts commencing on this date will be for a term of three (3) years
terminating on 31.3.2013. The consortium reserves the right to extend all
the contracts awarded by one (1) year (with the agreement of each
provider) should the consortium subsequently decide to do so.
The consortium intends to re advertise this contract every six months (in
April and October of each year) until October 2012. Accordingly potential
providers will be able to respond to these advertisements and submit
applications for accreditation to provide the services every six months
until October 2012. Note that all contracts awarded will terminate on 31
March 2013 regardless of when a contract is entered into unless the
consortium decides to extend the contract (with the agreement of each
provider) by one (1) year. At this stage of the procurement, the intention
is that there will be no limit on the number of providers that will be
permitted to enter the market and offered a contract to carry out the
services in any PCT area.
As indicated above, some or all of the eleven additional West Midlands
Primary Care Trusts may decide to participate in the procurement at some
later point. Where one of the eleven additional Primary Care Trust wishes
to procure CDSM programmes it will be named in an advertising round (i.e.
in April or October of the relevant year) and new providers will be
permitted to submit an accreditation application to run programmes in the
new PCT area in addition to the existing PCT areas. Existing providers
(i.e. providers who are already accredited) that wish to run programmes in
the new PCT area will also be required to submit an accreditation
questionnaire to run programmes in the new PCT area should they wish to do
so. Contracts to run CDSM programmes in a new PCT area will be awarded to
newly-accredited providers and existing providers at the same time.

2010, the deadline for receipt of completed accreditation questionnaires
is 17:00 on 30.11.2009.

II.2) QUANTITY OR SCOPE OF THE CONTRACT
II.2.1) Total quantity or scope: The estimated value of the services
covered by this procurement is in the region of GBP 200 000 – GBP 500 000
per annum. However, this figure may vary depending on take-up of the
service and the addition of other PCTs to the consortium. The estimated
spend of each new PCT that joins the consortium will be included in the
contract notice advertising their inclusion.
Estimated value excluding VAT:
Range: between 200 000 and 500 000 GBP.
II.2.2) Options: Yes.
Description of these options: The contract between the Provider and the
contracting authorities will be for a period of up to 3 years (depending
on when the contract is awarded) with an option to extend (exercisable
during the initial term) for a further year.

SECTION VI: COMPLEMENTARY INFORMATION
VI.3) ADDITIONAL INFORMATION: The publication of this contract notice is
non-mandatory as this is a Part B service. The procedure is based on the
open procedure although the contracting authority reserves the right to
deviate from such procedure as this service is a Part B service and the
requirement to follow the open procedure (or other OJEU procedure) is
non-mandatory.
Section I.1 –WMSHA is administering this procurement process on behalf of
the following contracting authorities (and their statutory successors and
organisations created as a result of re-organisation or organisational
changes):
— Coventry,

— South Staffordshire,
— Stoke-on-Trent,
— Telford & Wrekin and,
— Walsall.
In addition eleven other West Midlands PCTs (North Staffordshire,
Warwickshire, Herefordshire, Dudley, Wolverhampton, Solihull, Heart of
Birmingham, South Birmingham, Birmingham East and North, Sandwell and
Worcestershire) may join the consortium in which case the value and scope
of the contracts for services will increase accordingly. The inclusion of
any of these eleven PCTs (including their estimated spend) will be
advertised in the contract notice which advertises their inclusion in the
consortium.
Sections II.2.2 and II.3 - Contracts will be awarded for the provision of
these services from 1 April 2010. Contracts commencing on this date will
be for a term of three (3) years terminating on 31 March 2013. The
consortium reserves the right to extend all the contracts awarded by one
(1) year (with the agreement of each provider) should the consortium
subsequently decide to do so.
