6.Certificat constatator emis de Oficiul Registrului Comertului pentru persoane juridice romane in original sau copie legalizata, sau Autorizatie de functionare pentru persoane fizice romane in original sau copie legalizata;

1.Declaratie privind lista principalelor servicii similare executate în ultimii 3 ani împliniti la data depunerii ofertei;
2.Experienta similara;
3.Declaratie referitoare la efectivele medii anuale ale personalului angajat si al cadrelor de conducere în ultimii 3 ani;
4.Informatii privind asigurarea cu personal de specialitate ce poate fi disponibilizat;

6.Declaratie referitoare la utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru îndeplinirea corespunzatoare a contractului de servicii;
7.Informatii privind subcontractantii;

9.Certificat pentru sistemul calitatii, aplicat pe baza standardelor SR EN ISO seria 9000 pentru servicii de proiectare pentru infrastructura de transport, sau echivalent;
10.certificat OHSAS 18001:1999 sau echivalent.
Minimum level(s) of standards possibly required
1.Ofertantii vor face dovada finalizarii în ultimii 3 ani a doua contracte de servicii similare, cod CPV 71322500-6 Servicii proiectare tehnica pentru infrastructura de transport, a carei valoare cumulata sa fie de cel putin de 200000 lei RON fara TVA. Totodata se vor anexa copii dupa contractele de servicii si procesele verbale de predare - primire/receptie a documentatiilor. NU SE ACCEPTA DRUMURI PIETRUITE. 2. Recomandari din partea unor beneficiari/clienti privind prestarea serviciilor similare: se vor prezenta minim 2 (doua) recomandari din partea beneficiarilor. 3.Se cere asigurarea cu minim: - 2 ingineri specialitatea C.F.D.P; -1 topograf; -1 geolog; -1 verificator proiect; - Seful de proiect propus pentru îndeplinirea contractului va avea experienta în domeniul drumurilor si podurilor, în servicii de proiectare si/sau în supervizare de lucrari de constructii de minim 5 ani, demonstrata cu cartea de munca sau contract de munca; - Proiectantul de specialitate propus pentru îndeplinirea contractului va avea experienta în domeniul drumurilor si podurilor, în servicii de proiectare si/sau în supervizare de lucrari de constructii de minim 3 ani demonstrata cu cartea de munca sau contract de munca; 4.Se solicita dotare cu: - soft cu licenta în domeniul proiectarii de drumuri si poduri; - echipament ridicari topo; - soft prelucrare topo.
