formular 12 A Declaratii privind eligibilitatea; prezentare formular 12 B Declaratie privind neîncadrarea în prevederile art. 181; Certificat constatator eliberat de Ministerul Justitiei - O.R.C/echivalent din care sa rezulte numele complet, sediul, persoanele autorizate/administratori; Adresa de la Instanta Competenta/Curtea Districtuala/Agentie Guvernamentale unde îsi are sediul compania/sucursala sau echivalent din care sa rezulte ca operatorul economic nu este în insolventa/faliment/aranjament cu creditorii; Certificat de atestare fiscala eliberat de organul de administrare fiscala al unitatii administrativ teritoriale de pe raza careia societatea îsi are sediul social privind plata obligatiilor la bugetul de stat sau echivalent. Certificatul trebuie sa fie valabil la data deschiderii ofertelor; Certificat privind plata impozitelor si taxelor locale, valabil la data deschiderii ofertelor; Certificat pentru plata CAS (pentru statele în care autoritatile fiscale nu colecteaza si CAS) valabil la data deschiderii ofertelor; Cazier judiciar pentru administrator(i) asa cum sunt mentionati în Certificatul Constatator precum si pentru persoana împuternicita (unde este cazul) ce semneaza declaratia referitoare la neîncadrarea în art.180 si art.181, respectiv Declaratie privind eligibilitatea; Cazier judiciar al companiei (pt statele unde e aplicabil); Ofertantul trebuie sa faca dovada ca are codul CAEN asociat obiectului contractului

- Media Cifrei de afaceri a ofertantului pe ultimii trei ani (2006, 2007 si 2008) trebuie sa fie mai mare sau egala cu suma de 10.000.000 RON
