Experienta similara minima impusa este de 100.000 lei si va putea fi dovedita prin prezentarea unor contracte de servicii similare cu obiectul achizitiei, incheiate si executate in ultimii trei ani.
Se vor prezenta procesele verbale de receptie, copie dupa contracte precum si certificarile de buna executie (recomandari ) de la beneficiari.
Se solicita prezentarea listei conform Formular 12I privind asigurarea cu personalul de specialitate care este considerat strict necesar pentru îndeplinirea contractului.
Se solicita dupa completarea Formular 12G cu subcontractantii care realizeaza mai mult de 10% din contract.Autorizare pentru executia serviciilor deratizare-dezinsectie emisa de Inspectoratul de Politie Sanitara Teritorial, sau echivalent
Autorizatie de Mediu; sau echivalent
Autorizatie Sanitar Veterinara,sau echivalent
Adeverinta de atestare a personalului operator eliberata de Directia de Sanatate Publica, sau echivalent
Autorizatie de protectie a muncii de la Inspectoratul Teritorial de Protectie a Muncii,sau echivalent.
