1.Decl privind eligibilitatea completata în conf cu Form A din Sect Form - în orig;2.Decl privind neîncadrarea în situatiile prevazute la art. 181 din OUG 34/2006 completata în conf cu Form B din Sect Form - în orig;3.Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat (sanatate, pensii, somaj), val la data deschid of, elib de DGFPCFS - în orig sau copie legalizata.4.Certificat privind plata taxelor si impozitelor locale, val la data deschid ofertei, elib de Primaria locala- în orig sau copie legalizata.Pt persoane juridice române:Certificat constatator, în original sau copie legalizata emis, cu cel mult 30 zile inainte de data deschiderii ofertelor, de ORC de pe lânga Tribunalul teritorial, din care sa rezulte ca:- domeniul de activitate al ofertantului corespunde obiectului procedurii;- nu sunt înscrise mentiuni cu privire la aplicarea Legii nr. 85/2006 privind procedura insolventei, sau ca societatea se afla în incapacitate de plata, in original sau copie legalizata.Pt pers juridice straine: Documente edificatoare care sa dovedeasca o forma de inregistrare ca persoana juridica sau de inregistrare/atestare ori apartenenta profesionala, in conformitate cu prevederile legale din tara in care ofertantul este rezident. Documentele se vor prezenta in copie legalizata, anexandu-se si traducerile autorizate in limba romana.

a)lista a principalelor servicii similare prestate in ultimii 3 ani, continand valori, perioade de prestare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati. Prestarile de servicii se confirma prin prezentarea unor certificate/documente emise sau contrasemnate de o autoritate ori de catre clientul privat beneficiar.Se va completa si depune, în original, Formularul C2 din Sectiunea Formulare. b)declaratie pe propria raspundere referitoare la studiile, pregatirea profesionala si calificarea persoanelor responsabile pentru îndeplinirea contractului de servicii - în original; c)declaratie pe propria raspundere referitoare la efectivele medii anuale ale personalului angajat si al cadrelor de conducere în ultimii 3 ani - în original; Se va completa si depune, în original, Formularul F din Sectiunea Formulare.d)declaratie pe proprie raspundere privind masurile de protectie a mediului pe care ofertantul le poate aplica in timpul îndeplinirii contractului de servicii- în original;e)declaratie pe propria raspundere privind asigurarea conditiilor de munca si protectia muncii- în original;f)informatii privind partea din contract pe care operatorul economic are, eventual, intentia sã o subcontracteze - nu se prezinta daca ofertantul nu subcontracteaza;g)Standarde de asigurare a calitatii : Se vor prezenta, în copie certificate care atesta implementarea sistemului de calitate ISO 9001 sau documente edificatoare ca se afla in curs de implementare.
